阿尔茨海默病患者营养状况与认知功能受损程度的相关性研究

摘　　要：目的探讨阿尔茨海默病(AD)患者营养状况的变化及其与认知功能的相关性。方法收集AD患者125例,根据临床痴呆评定量表(CDR),将AD患者分为轻度28例,中度42例,重度55例,分别通过微型营养评定法(MNA-SF)及测定血清白蛋白、血红蛋白来评价患者的营养状况,并与对照组40例做对照研究;比较不同痴呆程度AD患者MNA-SF得分与血清白蛋白、血红蛋白浓度的变化。结果中度、重度AD组MNA-SF得分及血清白蛋白水平与对照组比较明显降低(P<0.05),重度AD组血红蛋白浓度较其他组别明显降低(P<0.05)。多元线性回归分析结果显示,MMSE得分与MNA-SF得分、血红蛋白浓度呈正相关(r值分别为0.485、0.228,P<0.05);Logistic回归分析结果显示,MNA-SF得分为AD患者痴呆程度的独立保护因子(P<0.05)。结论AD患者存在营养状况的变化,且与其病情的严重程度密切相关,在诊疗过程中应注重AD患者的营养状况。Objective To explore correlation between the nutriture and cognitive function impairment in patients with Alzheimer's disease(AD).Methods A total of 125 patients with AD were divided into mild(28 cases),moderate(42cases)and sever(55cases)groups according to clinical Dementia Rating Scale(CDR);meanwhile,40 healthy elderly people were also selected as the control group.The levels of their serum albumin and hemoglobin were tested at fasting state and Short-form Mini-Nutritional Assessment(MNA-SF)was evaluated.Results Levels of serum albumin and scores of MNA-SF in moderate and sever groups were significantly lower than those in control group(P<0.05),the hemoglobin level was significantly lower than those in other groups(P<0.05).There's a positive correlation between the scores of mini-mental State Examination(MMSE)and MNA-SF,between scores of MMSE and levels of hemoglobin according to the multiple linear regression analysis(r=0.485,r=0.228,P<0.05).The scores of MNA-SF was an independent protective factor according to the logistic regression analysis(P<0.05).Conclusion Changes of nutriture occurred in patients with AD.There's a close correlation between the nutriture and the severity of cognitive impairment.More emphases should be put for the nutriture of patients with AD.
