摘 要:目的探讨甲状腺转录因子-1(TTF-1)、微小RNA-25(miR-25)在肺癌患者中表达及与预后关系。方法选取2017年10月-2019年10月河南省新乡市中心医院收治的184例肺癌患者作为研究组,同期选取161例肺良性疾病作为对照组。统计两组及研究组不同生存状况患者TTF-1、miR-25表达,分析上述指标与临床病理的关系,采用受试者工作特征曲线(ROC)及ROC下面积(AUC)分析TTF-1、miR-25单一及联合预测死亡的价值,采用卡普兰-迈耶曲线(KM)生存曲线分析TTF-1、miR-25高危者、低危者生存曲线。结果研究组TTF-1 m RNA、miR-25 m RNA水平高于对照组(P<0.05);研究组中TTF-1、miR-25高表达者分化程度、淋巴结转移、临床分期与低表达者比较,差异有统计学意义(P<0.05);研究组生存者TTF-1、miR-25水平低于死亡者(P<0.05);TTF-1预测预后的AUC为0.799,当其截断值为1.06时,敏感度及特异度分别为75.00%、78.75%(P<0.05);miR-25预测预后的AUC为0.780,当其截断值为8.46时,敏感度及特异度分别为80.00%、71.87%(P<0.05);TTF-1+miR-25预测预后的AUC为0.857,敏感度及特异度分别为75.00%%、88.12%(P<0.05);TTF-1、miR-25高危者1年生存率低于低危者,差异有统计学意义(P<0.05)。结论肺组织中TTF-1 m RNA、miR-25 m RNA表达水平与肺癌分化程度、淋巴结转移、临床分期密切相关,高表达的TTF-1 mRNA、miR-25 mRNA不利于患者预后,有望成为预测肺癌患者预后的生物学标志物。Objective To explore the expression of thyroid transcription factor-1(TTF-1)and microRNA-25(miR-25)in lung cancer patients and their relationships with prognosis.Methods 184 patients with lung cancer admitted in Xinxiang Central Hospital from October 2017 to October 2019 were selected as the research group,and 161 benign lung diseases were selected as the control group during the same period.The expression levels of TTF-1 and miR-25 in the tissues of patients with different survival conditions in the two groups and the study group were counted,and the relationships between the above indicators and clinical pathology were analyzed.Receiver operating characteristic curve(ROC)and area under ROC(AUC)were used to analyze the value of single and joint predicted deaths of TTF-1 and miR-25;the survival curves of high-risk and low-risk TTF-1 and miR-25 were analyzed by Kaplan-Meyer curve(KM).Results The levels of TTF-1 mRNA and miR-25 mRNA in the research group were higher than those in the control group(P<0.05);the differentiation degree,lymph node metastasis,and clinical stage of patients with high expression levels of TTF-1 and miR-25 were worse than those patients with low expression levels of TTF-1 and miR-25(P<0.05);the levels of TTF-1 and miR-25 of the survivors in the study group were lower than those of the dead(P<0.05).The AUC of TTF-1 for prognosis was 0.799,with the cutoff value of 1.06,sensitivity of 75.00%and specificity of 78.75%(P<0.05);the AUC of miR-25-was 0.780,with the cutoff value of 8.46,sensitivity of 80.00%and specificity of 71.87%(P<0.05).The AUC of TTF-1+miR-25-was 0.857,and the sensitivity and specificity were 75.00%and 88.12%,respectively(P<0.05).The 1-year survival rate of TTF-1 and miR-25 high-risk individuals was lower than that of low-risk individuals(P<0.05).Conclusions The expression levels of TTF-1 mRNA and miR-25 mRNA in lung tissue were closely related to the degree of lung cancer differentiation,lymph node metastasis and clinical stage.The high expression levels of TTF-1 mRNA and miR-
