摘 要:腹膜假黏液瘤(PMP)是一种罕见的临床肿瘤综合征,早期诊断困难,手术切除为主要治疗手段,但手术后极易复发,准确的病理组织学分型有助于评估肿瘤生物学行为、辅助治疗决策、判断临床预后。数字病理的兴起,推动了肿瘤病理学由定性诊断向计算机辅助的定量诊断发展,肿瘤组织学图像分析由病理医师的选择性分析向全切片扫描的自动化分析发展,逐步实现全切片扫描、全景信息获取、全维度病理信息挖掘、全流程人工智能解读,构建了全新的精准病理诊断平台,已在乳腺癌、结直肠癌等肿瘤病理诊断中进行成功尝试。PMP的病理图像分析相关研究起步较晚,目前仍处于半定量阶段,技术方法简单粗糙,核心算法仍待探索。亟须建立计算机辅助的PMP病理图像定量分析,以降低病理医生的阅片负担并提高诊断准确性,有效提高病理学诊断对PMP预后的预测价值。As a rare clinical cancer syndrome,pseudomyxoma peritonei(PMP)is difficult to diagnose at early stage.The main treatment is surgical resection,but it is very easy to relapse after surgery.Accurate pathological classification is essential for evaluating tumor biological behavior,assisting treatment decision-making,and predicting clinical prognosis.The rise of digital pathology has promoted the changes of tumor pathology from qualitative diagnosis to computer aided quantitative diagnosis.As a result,tumor histopathology image analysis developed from pathologists′selective analysis to automated analysis based on whole slide imaging,gradually realized full slice scanning,access to panoramic information,full dimension exploration of pathological information and the whole process of artificial intelligence,building a new platform for the accurate pathological diagnosis,which has achieved successful attempt in breast cancer,colorectal cancer etc.However,the study of pathological image analysis of PMP has been slow and is still at the semi-quantitative stage.The technical method is simple and crude,while the core algorithm still needs to be explored.It is urgent to establish computer-aided quantitative analysis of pathological images,so as to reduce the burden of reading and improve the diagnostic accuracy of the pathologists,and effectively improve the prognostic value of pathological diagnosis for PMP.
