摘  要:目的:探讨缺氧诱导因子-1α(Hypoxia Inducible Factor-1α,HIF-1α)、上皮-间质转化(Epithelial-Mesenchymal Transition,EMT)标志蛋白:上皮标记钙黏素-E(E-cadherin)、波形蛋白(Vimentin)与血管生成拟态(Vasculogenic Mimicry,VM)在食管鳞癌中的表达、意义及关系。方法:对60例食管鳞癌组织、16例癌旁正常食管组织用免疫组化检测中HIF-1α、E-cadherin、Vimentin的表达情况,免疫组化/PAS双重染色法(Periodic Acid-Schiff,PAS)检测VM的表达情况。结果:HIF-1α在食管鳞癌组织和癌旁正常食管组织中的表达率分别为56.67%(34/60)和18.75%(3/16),差异具有统计学意义(χ2=7.269,P=0.007)。E-cadherin在食管鳞癌组织和癌旁正常食管组织中的表达率分别为28.33%(17/60)和62.50%(10/16),差异具有统计学意义(χ2=6.438,P=0.011)。Vimentin在食管鳞癌组织和癌旁正常食管组织的表达率分别为63.33%(38/60)和12.50%(2/16),差异具有统计学意义(χ2=13.092,P=0.000)。VM在食管鳞癌组织和癌旁正常食管组织的表达率分别为41.67%(25/60)和0.00%(0/16),差异具有统计学意义(χ2=9.935,P=0.001)。HIF-1α、E-cadherin、Vimentin及VM表达均与浸润深度、淋巴结转移和临床分期密切相关。相关分析显示在食管鳞癌组织中,HIF-1α与Vimentin、VM的表达呈正相关(r=0.591,P=0.000;r=0.534,P=0.000),E-cadherin与HIF-1α、Vimentin、VM的表达均呈负相关(r=-0.570,P=0.000;r=-0.366,P=0.004;r=-0.381,P=0.003),Vimentin与VM的表达呈正相关(r=0.433,P=0.001)。结论:在食管鳞癌中存在HIF-1α、EMT和VM高表达;HIF-1α、EMT及VM与食管鳞癌的侵袭转移相关;HIF-1α、EMT及VM的异常表达在食管鳞癌的发生及侵袭转移过程中具有协同作用。Objective:The study was designed to investigate the expression and relationship of hypoxia inducible factor-1a(HIF-1α),epithelial mesenchymal transition(E-cadherin and Vimentin),and vasculogenic mimicry in esophageal squamous cell carcinoma(ESCC).Methods:HIF-1α,E-cadherin,Vimentin protein expression were detected in 60 cases of ESCC and 16 cases of the adjacent normal tissue with immunohistochemistry.The presence of VM was detected with double stain of CD31 and PAS.Results:The positive rates of HIF-1αin ESCC and normal epithelial tissue were 56.67%(34/60)and 18.75%(3/16),respectively,with a significant difference between the two groups(χ2=7.269,P=0.007).The positive rates of E-cadherin in ESCC and normal epithelial tissue were 28.33%(17/60)and 62.50%(10/16),respectively,with a significant difference between the two groups(χ2=6.438,P=0.011).The positive rates of Vimentin in ESCC and normal epithelial tissue were 63.33%(38/60)and 12.50%(2/16),respectively,with a significant difference between the two groups(χ2=13.092,P=0.000).The positive rates of VM in ESCC and normal epithelial tissue were 41.67%(25/60)and 0.00%(0/16),respectively,with a significant difference between the two groups(χ2=9.935,P=0.001).HIF-1α,E-cadherin,Vimentin and VM was closely correlated with depth of invasion,clinical stages and lymphatic metastasis in ESCC.Bivariate analysis showed that HIF-1a was positively correlated with VM and Vimentin in ESCC(r=0.591,P=0.000;r=0.534,P=0.000).E-cadherin was negatively correlated with HIF-1α,Vimentin and VM(r=-0.570,P=0.000;r=-0.366,P=0.004;r=-0.381,P=0.003).Vimentin was positively correlated with VM(r=0.433,P=0.001).Conclusions:VM,EMT and high HIF-1αeppression exist in ESCC.The ESCC with VM,EMT and high HIF-1αexpression have a important role in invasion and metastasis.Abnormal expressions of HIF-1α,EMT and VM have a synergistic effect on the occurrence,invasion and metastasis in ESCC.
