摘 要:目的分析结合PICOT原则的临床路径教学法在风湿免疫科住院医师规范化培训中的应用效果。方法选取2016年1月~2019年12月在苏州大学附属第二医院风湿免疫科进行规范化培训轮转的60名住院医师为研究对象,按随机数字表法随机分为PICOT-CP组(30名)和传统教学组(30名),分别应用PICOT-CP教学法及传统教学法对两组学生进行带教,最后结合学生出科考核成绩(包括理论成绩、技能成绩、病历书写成绩、诊断与处理能力四个项目)及满意度调查问卷(包括加深对疾病了解、提高独立思考能力、分析处理疾病能力、科研能力、沟通能力五个项目)的形式进行效果评估。结果PICOT-CP组出科理论成绩、技能成绩、病历书写成绩、诊断与处理能力均高于传统教学组,差异均有统计学意义(P=0.000,P=0.000,P=0.000,P=0.000)。同时记录两组学生满意度情况,包括加深对疾病了解、提高独立思考能力、分析处理疾病能力、科研能力、沟通能力五个项目,结果显示:PICOT-CP组满意度(上述五项)明显高于传统教学组,差异均有统计学意义(P=0.032,P=0.038,P=0.023,P=0.027,P=0.012)。结论PICOT-CP教学法应用于风湿免疫科教学有利于提高教学质量,增强住院医师的临床综合能力,提高教学满意度。Objective To analyze the application effect of clinical pathway teaching method combined with PICOT principle in standard training of residents in the department of rheumatology and immunology.Methods A total of 60 residents who were undergoing standard training rotation in the department of rheumatology and immunology,the Second Affiliated Hospital of Soochow University,from January 2016 to December 2019 were selected as the research objects.They were divided into the PICOT-CP group(n=30)and the conventional teaching group(n=30)according to the random number table method.Meanwhile,the PICOT-CP teaching method and conventional teaching method were applied to teach the two groups of students respectively.Finally,the effect was evaluated by the form of combining the students'examination scores(including theoretical scores,skill scores,medical record writing scores,diagnosis and treatment ability)and satisfaction questionnaire(including deepening the understanding of diseases,improving independent thinking ability,analyzing and treating diseases,scientific research ability and communication ability).Results The theoretical scores,skill scores,medical record writing scores,diagnosis and treatment ability of the PICOT-CP group were higher than those of the conventional teaching group,and the differences were statistically significant(P=0.000,P=0.000,P=0.000,P=0.000).At the same time,students'satisfactions,including deepening the understanding of diseases,improving independent thinking ability,analyzing and treating diseases,scientific research ability and communication ability of the two groups of students were recorded.The results showed that the satisfactions of the PICOT-CP group(the above five items)were significantly higher than those of the conventional teaching group,and the differences were statistically significant(P=0.032,P=0.038,P=0.023,P=0.027,P=0.012).Conclusion The application of PICOT-CP teaching method in the teaching of rheumatology and immunology is conducive to improving the teaching quality,enhanci
