摘  要:目的通过对比帕金森病(Parkinson’s disease,PD)与阿尔茨海默病(Alzheimer disease,AD)患者肠道菌群差异,为患者提供治疗新思路。方法选取2017年1月至2020年1月就诊于四川大学华西医院神经内科的PD与AD患者为本次研究对象,对照组为PD患者,观察组为AD患者,按照粪便DNA试剂盒说明书提取,采用Illumina Miseq平台对细菌16S rRNA V3~V4区运用生物信息学的方法进行计算汇总,使用uclust进行聚类分析,优化分类相似度>97%归为一个操作分类单元(operational taxonomic units,OTU)。利用Greengenes数据库物种组成比例情况,抽取患者空腹静脉血3 ml,采用酶联免疫吸附法(ELISA)检测血清IL-1、IL-6、IL-8及TNF-α的含量。结果通过对高变区域序列两组患者序列对比,98例样品属于15个门,28个纲,70个科,202个属,221个种,1223个OTU。观察组中Chao、Ace、Shannon值降低(P<0.05),Simpson数值升高(P<0.05),在门的水平上有变形杆菌门(12%vs 6%)、拟杆菌门(30%vs 35%)、放线菌门(10%vs 14%)、厚壁菌门(32%vs 26%),与对照组相比,拟杆菌门比例上升(P<0.05),放线菌门比例有上升趋势。变形菌门、厚壁菌门比例下降(P<0.05);在属的水平差异分析,与对照组相比,布劳特氏菌属、梭杆菌属上升(P<0.05),双歧杆菌属及乳酸杆菌属下降。炎性因子研究中,与对照组相比,观察组患者IL-1、IL-6、IL-10、TNF-α的含量均升高(P<0.05)。结论AD患者肠道菌群主要由乳酸杆菌下降导致,并且与炎症因子相互作用,与患者病情发生相关,值得我们进一步研究。Objective To compare the intestinal microflora of patients with Parkinson’s disease(PD) and Alzheimer’s disease(AD), so as to provide new ideas for the treatment of PD.Methods From Jan. 2017 to Jan. 2020, patients with PD and AD were included in the study, the control group was PD, and the observation group was AD. According to the extraction of fecal DNA kit, using Illumina miseq platform to calculate and summarize the bacterial 16 SrRNA V3-V4 area, using the method of bioinformatics and uclust for cluster analysis, optimizing the classification similarity to be more than 97% into an operational taxonomic units(OTU). Green genes database was used for the proportion of species composition. IL-1, IL-6, IL-8 and TNF-α were measured by enzyme-linked immunosorbent assay(ELISA). Results 15 phylums, 28 classes, 70 families, 202 genus, 221 species and 1 223 OTUs were detected. In the observation group, Chao, Ace and Shannon decreased(P<0.05), Simpson increased(P<0.05), proteus(12% vs 6%), Bacteroides(30% vs 35%), actinomycetes(10% vs 14%), pachytenella(32% vs 26%), compared with the control group, the ratio of Bacteroides increased(P<0.05), and the ratio of actinomycetes increased. The proportion of Proteus and posterior wall decreased(P<0.05). Compared with the control group, the proportion of Brucella and Clostridium increased(P<0.05), and that of Bifidobacterium and Lactobacillus decreased. In the study of inflammatory factors, compared with the control group, the contents of IL-1, IL-6, IL-10 and TNF-α in the observationgroup were all increased(P<0.05). Conclusion The intestinal flora of AD patients is mainly caused by the decrease of lactobacillus, and the interaction with inflammatory factors is related to the occurrence of the disease, which is worthy of further study.
