摘 要:目的探讨血浆保存时间和制备方法对冷沉淀凝血因子(简称冷沉淀)质量的影响。方法随机选取2019年12月至2020年1月郑州地区采集的240袋400 mL规格的全血,根据采集到制备成新鲜冰冻血浆(FFP)的时间分为A组(8 h内,120袋)和B组(8~18 h内,120袋),每组随机选取60袋用恒温摇摆式水浴箱制备冷沉淀,60袋用全自动冷沉淀制备仪制备冷沉淀。比较不同制备方法和不同保存时间的FFP制备出的冷沉淀质量。结果对于A组,两种方法制备的冷沉淀均符合国家标准,制备仪组凝血因子Ⅷ(FⅧ)含量符合率、纤维蛋白原(Fib)含量高于水浴箱组(P<0.05),两组FⅧ含量、Fib含量符合率差异无统计学意义(P>0.05)。对于B组,采用恒温水浴箱制备的冷沉淀部分符合国家标准,采用全自动冷沉淀制备仪制备的冷沉淀全部符合国家标准,制备仪组FⅧ含量、FⅧ含量符合率、Fib含量、Fib含量符合率均高于水浴箱组(P<0.05)。根据冷沉淀制备仪制备结果,A组冷沉淀FⅧ含量、FⅧ含量符合率、Fib含量符合率高于B组(P<0.05),两组Fib含量差异无统计学意义(P>0.05)。结论为保证冷沉淀质量,采血后应尽可能在8 h内制备出FFP,且采用全自动冷沉淀制备仪制备的冷沉淀质量更佳。Objective To investigate the effects of plasma storage time and preparation method on the quality of cryoprecipitate.Methods A total of 240 bags of 400 mL whole blood collected in Zhengzhou from December 2019 to January 2020 were randomly selected and divided into group A(in 8 hours,120 bags)and group B(8 to 18 hours,120 bags)according to the time between blood collection and preparation of fresh frozen plasma(FFP).In each group,60 bags of FFP were randomly selected to prepare cryoprecipitate with constant temperature rocking water bath,and 60 bags of cryoprecipitate were prepared with a fully automatic cryoprecipitator.The quality of cryoprecipitate prepared by different preparation methods and from FFP of different storage time was compared.Results For group A,the cryoprecipitate prepared by the two methods both met the national standards.The blood coagulation factorⅧ(FⅧ)content coincidence rate and fibrinogen(Fib)content in the preparation instrument group were higher than those in the water bath group(P<0.05).There was no statistical difference in the FⅧcontent coincidence rate and Fib content between the two groups(P>0.05).For group B,part of the cryoprecipitate prepared by the constant temperature water bath met the national standard,while the cryoprecipitate prepared by the automatic cryoprecipitator all met the national standard.FⅧcontent,FⅧcontent coincidence rate,Fib content,and Fib content coincidence rate in the preparation instrument group were all higher than those in the water bath group(P<0.05).According to the results of cryoprecipitate preparation method,FⅧcontent,FⅧcontent coincidence rate,and Fib content in the group A were all higher than those in the group B(P<0.05),and there was no statistical difference in Fib content between the two groups(P>0.05).Conclusion In order to ensure the quality of cryoprecipitate,FFP should be prepared within 8 hours after blood collection,and the quality of cryoprecipitate prepared by automatic cryoprecipitator is better.
