摘 要:目的使用Meta分析评估机器人手术和胸腔镜手术治疗早期非小细胞肺癌的临床效果。方法利用计算机在PubMed、Embase、万方、知网等数据库中进行检索,检查时限为建库至2020年5月,对筛选文献进行质量评价,利用RevMan5.3软件对纳入文献进行Meta分析。结果共纳入8篇文献,在早期非小细胞肺癌的治疗中,机器人手术比胸腔镜手术术中切除淋巴结数量更多[MD=2.84,95%CI(0.37,5.32),P=0.02],出血量更少[MD=-53.31,95%CI(-90.20,-16.42),P=0.005],总费用更多[MD=40.01,95%CI(35.71,44.31),P<0.00001]。两种手术方式在术后并发症、手术时间、术后住院时间、术后引流时间、中转开胸方面无统计学差异(P>0.05)。结论在临床早期非小细胞肺癌的治疗中,机器人手术相比胸腔镜手术具有一定优势,相信机器人手术将更多应用于临床早期肺癌的治疗。Objective To evaluate the clinical effect of robotic surgery and thoracoscopic surgery in the treatment of early stage non-small cell lung cancer by meta-analysis.Methods The computer is used to search in PubMed,Embase,Wanfang,CNKI and other databases,and the retneval time is from the database construction to May 2020.After screening the relevant literature,the quality evaluation will be carried out,and the literatures finally included performed eeta analysis by RevMan5.3 software.Results A total of 8 articles were included.In the treatment of early stage non-small cell lung cancer,robotic surgery has more lymph nodes removed[MD=2.84,95%CI(0.37,5.32),P=0.02],less blood loss[MD=-53.31,95%CI(-90.20,-16.42),P=0.005]and more total cost[MD=40.01,95%CI(35.71,44.31),P<0.00001]than thoracoscopic surgery.There were no significant differences in postoperative complications,operation time,hospitalization time,drainage time and transit thoracotomy between the two surgical methods(P>0.05).Conclusion In the treatment of clinical early stage non-small cell lung cancer,robotic surgery has certain advantages over thoracoscopic surgery.It is believed that robot surgery will be more used in the treatment of early clinical lung cancer.
