醛固酮腺瘤患者应用肾上腺全切除术、保留肾上腺手术的临床观察

摘　　要：目的探究醛固酮腺瘤患者应用肾上腺全切术、保留肾上腺手术的效果。方法回顾性分析2016年3月至2019年3月濮阳市油田总医院收治的70例醛固酮腺瘤患者临床资料,其中36例行腹腔镜下肾上腺全切除术(肾上腺全切组),34例行腹腔镜下肾上腺部分切除术(保留肾上腺组),比较两组手术情况,并随访患者术后血压恢复以及疾病复发情况。结果两组的手术用时、术中出血、住院周期、术后收缩压、舒张压、血钾以及醛固酮等方面比较,无显著差异(P>0.05),保留肾上腺组术后并发症发生率显著低于肾上腺全切组(P<0.05)。随访6个月,肾上腺全切组患者的术后血压有效率为88.89%,保留肾上腺组为85.29%;肾上腺全切组复发率为0.00%,保留肾上腺组为5.88%,两组术后血压有效率、复发率比较,无显著差异(P>0.05)。结论与肾上腺全切术相比,保留肾上腺手术在醛固酮腺瘤患者的治疗中尽管存在复发的可能性,但其并发症轻且少,安全可靠。Objective To investigate the effect of total adrenalectomy and adrenalectomy in patients with aldosteroneproducing adenoma.Methods The clinical data of 70 patients with aldosterone adenoma admitted to Puyang Oilfield General Hospital from March 2016 to March 2019 were analyzed retrospectively.36 patients underwent total laparoscopic adrenalectomy(total adrenalectomy group),laparoscopic partial adrenalectomy was performed in 34 patients(adrenal sparing group).Postoperative blood pressure and disease recurrence were compared between the two groups.Results There was no significant difference in the operation time,intraoperative bleeding,hospitalization period,postoperative systolic blood pressure,diastolic blood pressure,blood potassium and aldosterone between the two groups(P>0.05),the incidence of postoperative complications in adrenalectomy group was significantly lower than that in total adrenalectomy group(P<0.05).The effective rate of postoperative blood pressure was 88.89%in the total adrenalectomy group and 85.29%in the adrenalectomy group,and 0.00%in the adrenalectomy group and 5.88%in the adrenalectomy group,there was no significant difference(P>0.05).Conclusion Compared with total adrenalectomy,adrenalectomy is safe and less complicated in the treatment of patients with aldosterone-producing adenoma.
