摘 要:目的了解鸡蛋膜活性肽对食管癌患者放化疗期间的营养免疫效应。方法选取2019年2月至10月淮安市第一人民医院和淮安市肿瘤医院肿瘤内科、放疗科患者116例,随机分为常规营养治疗组(TNSG组)和肽强化支持组(PNSG组),每组58例。PNSG组采用鸡蛋膜活性肽强化制剂与TNSG组采用能全素进行口服营养补充。两组患者均在入选次日、第7天、第14天进行血常规、空腹血糖、血白蛋白、血清前白蛋白及IgG、IgM、IgA检测,常规进行人体成分分析和体格测量。新辅助化疗与放疗期间给予口服营养补充,观察血液营养免疫指标变化。结果血清白蛋白、前白蛋白指标在试验7~14 d改变差异无统计学意义(t_(7~14)=-0.298,P=0.381;t_(7~14)=-2.644,P>0.05);在试验0~7 d(t_(0~7)=3.782,P<0.001;t_(0~7)=15.834,P<0.001)、在试验0~14 d(t0~14=3.484,P<0.001;t0~14=13.189,P<0.001),血清总蛋白和血清前白蛋白指标差异均有统计学意义。上臂围在试验0~7 d(t_(0~7)=1.037,P<0.001),在试验0~14 d(t0~14=0.659,P=0.001)阶段差异均有统计学意义;而瘦体组织在试验0~7 d(t_(0~7)=0.255,P>0.05)差异无统计学意义,在试验7~14 d(t_(7~14)=0.292,P<0.05)、在试验0~14 d(t0~14=0.547,P<0.05)间,两组间差异也无统计学意义。在免疫指标IgG(t_(0~7)=0.006,P>0.05)、IgM(t_(0~7)=0.032,P<0.001)血液浓度在7 d内差异无统计学意义;该指标7 d和14 d时差异均有统计学意义;而血液IgA浓度自试验后在7 d、14 d差异均有统计学意义。结论鸡蛋膜活性肽在食管癌放化疗治疗中具有一定的营养免疫调节作用。Objective To understand egg membrane peptides can strengthen nutrition support to patients suffering from esophageal cancer during their concurrent chemoradiation treatment.Methods A total of 116 patients from the Department of Radiotherapy and the Department of Oncology,Huai′an First People′s Hospital and Huai′an Cancer Hospital from February to October 2019 were selected and divided into the routine nutritional support group(TNSG group)and peptide nutrition support group(PNSG group)by a randomized,single-blind method,with 58 patients in each group.Routine tests of fasting blood glucose,serum albumin,serum prealbumin and IgG,IgM,IgA were performed on the next day,7th and 14th day,and routine body composition analysis and physical measurement were performed.Results There was no statistical difference in the changes of serum albumin and prealbumin indexes during the 7-14 days(t_(7~14)=-0.298,P=0.381;t_(7~14)=-2.644,P>0.05);before the test and the 7th day(t_(0~7)=3.782,P<0.001;t_(0~7)=15.834,P<0.001),between the test and the 14th day(t0~14=3.484,P<0.001;t0~14=13.189,P<0.001).There were statistical differences between the two groups in terms of serum total protein and serum prealbumin indicators.The upper arm circumference was statistically different during the period before the test to 7 days(t_(0~7)=1.037,P<0.001),and the period before the test to 14 days(t0~14=0.659,P=0.001);while the lean body mass was in the test No statistical difference was seen from the first 7 days(t_(0~7)=0.255,P=0.130),and no statistical difference was seen between the test 7-14 days(t_(7~14)=0.292,P=0.045)and 14 days before the test(t0~14=0.547,P=0.05),there were statistical differences and clinical significance between the two groups.In the immune index IgG(t_(0~7)=0.006,P>0.05),IgM(t_(0~7)=0.032,P<0.001)blood concentration within 7 days,there was no statistical difference between the two groups.There was statistical difference in this index at 14 days and at 7 days.The blood IgA concentration was statistically different at 7 days
