摘 要:目的分别测定不同孕周母亲及其所生新生儿的血氨基酸含量,通过代谢组学分析不同孕周母亲及新生儿血氨基酸代谢谱的差异,以期为临床营养支持提供理论依据。方法收集2017年11月至2020年7月佛山市三水区妇幼保健院及中山大学附属第六医院产科、新生儿科不同孕周母亲及其所生新生儿的指尖/足跟末梢血液标本,共143例,母亲体健,无妊娠合并症,胎儿宫内发育良好,根据孕周分为3组:<32周(22例),32~36+6周(59例),≥37周(62例),利用串联质谱技术及定量软件测定血样本中氨基酸代谢产物的含量,同时运用正交偏最小二乘判别分析法对3组血代谢产物进行代谢组学分析并寻找重要的差异物质。结果(1)早产与足月的母亲及新生儿血氨基酸代谢谱差异有统计学意义;(2)在代谢组模式识别结合变量权重图分析发现,母亲组有6个权重较大的物质,孕<32周:丙氨酸(139.12±20.00)μmol/L、甘氨酸(129.52±35.00)μmol/L、谷氨酸(193.51±25.52)μmol/L、脯氨酸(99.94±18.92)μmol/L、缬氨酸(83.60±12.96)μmol/L、亮氨酸(93.82±20.06)μmol/L;孕32~36+6周:丙氨酸(137.59±27.72)μmol/L、甘氨酸(117.59±24.81)μmol/L、谷氨酸(188.05±40.25)μmol/L、脯氨酸(104.69±20.83)μmol/L、缬氨酸(84.76±13.11)μmol/L、亮氨酸(94.65±18.28)μmol/L;≥37周:丙氨酸(244.67±84.83)μmol/L、甘氨酸(165.76±61.67)μmol/L、谷氨酸(238.40±73.79)μmol/L、脯氨酸(137.41±41.31)μmol/L、缬氨酸(112.17±25.63)μmol/L、亮氨酸(112.62±35.94)μmol/L。与孕周不足月的母亲相比,孕周足月母亲的丙氨酸、甘氨酸、谷氨酸、脯氨酸、缬氨酸、亮氨酸水平升高,差异均有统计学意义(均P<0.05)。新生儿组有5个权重较大的物质,<32周:脯氨酸(431.63±235.66)μmol/L、谷氨酸(151.40±34.43)μmol/L、甘氨酸(144.84±48.13)μmol/L、丙氨酸(184.11±54.80)μmol/L、组氨酸(150.78±103.77)μmol/L;32~36+6周:脯氨酸(571.62±228.19)μmol/L、谷氨酸(177.98Objective To provide theoretical basis for clinical nutrition support determining the blood amino acids of and finding the differences of blood amino acid metabolic profiles of mothers at different gestational ages and their newborns by metabonomics analysis.Methods From November,2017 to July,2020,the blood samples from the fingertips or heel tips of the mothers at different gestational ages and their newborns at Sanshui District Maternal and Children Health Care Hospital and Sixth Hospital Affiliated to Sun Yat-sen University were collected,143 cases.The mothers were healthy,with no pregnancy complications.Their fetuses developed well.According to their gestational ages,they were divided into a<32 weeks group(n=22),a 32-36+6 weeks group(n=59),and a≥37 weeks group(n=62).Tandem mass spectrometry technology and quantitative software were used to determine the contents of amino acid metabolites in the blood samples;and orthogonal partial least square discriminant analysis was used to conduct metabolomic analysis of the blood metabolites of the three groups and to search for important different substances.Results There were statistical differences in serum amino acid metabolic profiles between the premature and full-term mothers and newborns.Metabolome pattern recognition analysis combined with variable importance plot analysis found that there were 6 important substances in the maternal group.The levels of alanine,glycine,glutamate,proline,valine,and leucine were(139.12±20.00)μmol/L,(129.52±35.00)μmol/L,(193.51±25.52)μmol/L,(99.94±18.92)μmol/L,(83.60±12.96)μmol/L,and(93.82±20.06)μmol/L in the<32 weeks'mother group,were(137.59±27.72)μmol/L,(117.59±24.81)μmol/L,(188.05±40.25)μmol/L,(104.69±20.83)μmol/L,(84.76±13.11)μmol/L,and(94.65±18.28)μmol/L in the 32-36+6 weeks'mother group,and were(244.67±84.83)μmol/L,(165.76±61.67)μmol/L,(238.40±73.79)μmol/L,(137.41±41.31)μmol/L,(112.17±25.63)μmol/L,and(112.62±35.94)μmol/L in the≥37 weeks'mother group;the levels were higher in the≥37 wee
