医生履行医患沟通伦理责任的影响因素及对策思考

摘　　要：患者在就医期间维持良好的心理状况会对病情康复有正性影响,而与医生的关系是影响患者心理状况的要素之一。因此,就医方而言,医生履行医患沟通的伦理责任极为重要。由于医生对医患沟通重要性认识不到位、个体性格和社会阅历的差异等主观因素以及负面社会舆论和患者非理性就医态度所导致的医生自我保护意识增强、医生缺乏有效沟通时间和医患沟通能力提升再培训的路径等客观因素,影响了医生履行医患沟通的伦理责任。应通过激发医生履行医患沟通伦理责任的内部心理动机和营造促进医生履行医患沟通伦理责任的外部支持环境,如切实提高医学生的人文素养并设置有效教育和考核的机制、缓解大型医院人流量过多的问题、增加医生医患沟通能力提升的再培训路径、引导患者正确应用知情同意权和自主决策权、呼吁媒体引导正确的道德风向等对策,促进医生履行医患沟通伦理责任,为构建和谐医患关系提供有益借鉴。Maintaining a good psychological state during medical treatment will have a positive impact on the patient’s condition,and the doctor-patient relationship with the doctor is one of the factors affecting the patient’s psychological status.Therefore,it is very important for doctors to fulfill the ethical responsibility of doctor-patient communication.Due to the subjective factors such as the doctor’s insufficient understanding of the importance of doctor-patient communication,the difference of individual personality and social experience,the objective factors such as the enhancement of doctors’self-protection consciousness caused by negative public opinion and patients’irrational medical attitude,the lack of effective communication time of doctors and the path of improving doctor-patient communication ability and retraining,which affects the doctors to fulfill the doctor-patient communication ethical responsibility.It is necessary to promote doctors to fulfill the ethical responsibility of doctor-patient communication,and provide useful reference for the construction of harmonious doctor-patient relationship by stimulating their internal psychological motivation and creating an external support environment for doctors to fulfill the ethical responsibility of doctor-patient communication,such as improving the humanistic quality of medical students and setting up effective education and assessment mechanism,alleviating the problem of excessive flow of people in large hospitals,increasing the retraining path of doctors-patient communication ability,guiding patients to correctly apply the right of informed consent and independent decision-making power,calling on the media to guide the correct moral direction,etc.
