摘  要:目的评价叙事医学对超声科住院医师共情能力和医患沟通能力的培训效果。方法将2020年5月1日-2020年8月31日在某院超声科参加规范化培训的30名住院医师随机分为观察组和对照组,每组15人。对照组接受常规的规范化培训,观察组在对照组基础上增加叙事医学相关培训。在培训前后,对观察组和对照组进行共情能力和医患沟通能力测试,观察组在培训后进行问卷调查。结果培训后,观察组的共情能力(118.00±6.53)分和医患沟通能力(150.33±10.29)分明显高于对照组(107.70±5.89、140.67±08.36)分,差异具有统计学差异,P<0.05;观察组培训3个月后共情能力(118.00±6.53)分和医患沟通能力(150.33±10.29)分高于培训前(105.73±5.93、135.60±10.27)分,差异具有统计学意义,P<0.05。观察组对人文素养提高程度均表示满意,其中非常满意为73.3%,满意为26.7%;叙事医学相关内容中平行病历书写对人文教育价值最高,其中非常有效为73.3%,有效为26.7%。结论叙事医学可以提高住院医师共情能力和医患沟通能力,促进其反思,提升其专业知识,对人文素养的培养具有重要意义。Objective To evaluate the training effect of narrative medicine on the empathy ability and doctor-patient communication ability for the residents of ultrasound department. Methods Thirty residents who participated in standardized training in ultrasound department of a hospital from May 1, 2020 to August 31, 2020 were randomly divided into observation group and control group, 15 residents in each group. The control group received the routine standardized training, while the observation group received additional narrative medical training on the basis of the control group. Before and after the training, the empathy ability test and doctor-patient communication ability test were both done by the observation group and the control group. After the training, a questionnaire survey was conducted for observation group. Results After training, the scores of empathy ability(118.00±6.53) and doctorpatient communication ability(150.33±10.29) in the observation group were significantly higher than those in the control group(107.70±5.89, 140.67±8.36). In the observation group, the scores of empathy ability(118.00±6.53) and doctor-patient communication ability(150.33±10.29) after 3 months training were significantly higher than those before training(105.73±5.93, 135.60±10.27). The observation group all satisfied with the improvement of humanistic quality, 73.3% were very satisfied and 26.7% were satisfied. Among the related contents of narrative medicine, the parallel medical records writing had the highest value in humanistic education, 73.3% were very effective and 26.7% were effective. Conclusion The narrative medicine improve the empathy ability and communication ability,promote the reflection and enhance the professional knowledge, which plays an important role on cultivating humanistic quality for resident.
