纤维肉瘤型隆突性皮肤纤维肉瘤1例

摘　　要：患者女,52岁,发现右肩部皮肤肿物半年,切除术后4个月,复发3个月余。皮肤科情况:右肩部可见一隆起于皮肤表面的半球形肿物,大小约2.5 cm×2 cm×1.8 cm,表面较光滑,可见多条树枝状毛细血管,压痛(-)。病理检查及免疫组织化学特点:经典隆突性皮肤纤维肉瘤区域由席纹状排列的梭形细胞构成,伴有纤维肉瘤转化区域由鱼骨状排列的梭形细胞构成,纤维肉瘤转化区域内肿瘤细胞CD34局灶阳性,p53(80%+),Ki-67(20%+),Bcl-2(+),EMA(-),S-100(-),HMB45(-),SMA(-),Desmin(-)。诊断:纤维肉瘤型隆突性皮肤纤维肉瘤。治疗:手术切除,拆线后愈合良好。A 52-year-old female patient presented with a mass on the right shoulder for about half a year,and recieved excision procedure four months ago,but relapsed half a month later.Physical examination revealed a nontender smooth semi-sphere protrusive lesion located on the right shoulder measuring 2.5 cm×2 cm×1.8 cm with lots of capillaries on its surface.Excisional biopsy showed storiform pattern of spindle cells in the classic DFSP area,and herringbone pattern of spindle cells in the fibrosarcomatous transformation area,in which immunohistochemical studies showed CD34 focal(+),p53(80%+),Ki-67(20%+),Bcl-2(+),EMA(-),S-100(-),HMB45(-),SMA(-),Desmin(-).Then the diagnosis of fibrosarcomatous dermatofibrosarcoma protuberans was made.An excision procedure was performed,afterwards suture was removed and the wound healed well.
