摘  要:目的分析两起食物中毒中副溶血性弧菌分离株的分子分型、耐药性特点,以揭示深圳市龙华区副溶血性弧菌的分子流行特征。方法对该两起食物中毒事件分离到的28株副溶血弧菌(其中5株分离自2019年5月15日食物中毒患者肛拭子,另外23株分离自2019年9月1日食物中毒患者肛拭子、厨师肛拭子、食物及环境样品)进行血清学分型,PCR检测耐热直接溶血毒素基因(tdh)、耐热相关溶血毒素基因(trh),脉冲场凝胶电泳(PFGE)分子分型分析及微量肉汤法检测抗生素敏感性。结果两起食物中毒副溶血弧菌分离株血清学均以O3∶K6型为主;抗生素敏感性分析显示,仅对头孢唑啉(CFZ)出现耐药或中度敏感,对其他抗生素均敏感;毒力基因PCR检测结果显示,均以tdh+、trh-型菌株为主;经限制性内切酶(NotⅠ)酶切后的DNA指纹图谱,两起食物中毒菌株间的相似值为68.3%~100.0%,按100%相似度可以分为21个型别。结论不同时间不同地点的食物中毒事件中分离株间具有高度的同源性,同时还发现存在不同血清型的菌株间的DNA指纹图谱存在紧密相关性,提示辖区存在副溶血弧菌的持续污染以及分子水平的变异。Objective Study on the molecular typing and antimicrobial resistance of Vibrio parahaemolyticus strains of two foodborne diseases,in order to explore the molecular epidemic characteristics of Vibrio parahaemolyticus in Longhua district,Shenzhen.Methods 28 strains of Vibrio parahaemolyticus isolated from the two food poisoning incidents were serologically typed(Five of them were isolated from anal swabs of food poisoning patients on May 15,2019,and the other 23 were isolated from suspicious foods,environment and the anal swabs of food poisoning patients and chefs on September 1,2019).The heat-resistant direct hemolysin gene(tdh)and heat-resistant hemolysin gene(trh)were detected by PCR.The isolates were molecular typed by pulse field gel electrophoresis(PFGE)and determined their antibiotic susceptibilities.Results The serotype of Vibrio parahaemolyticus strains of two foodborne diseases were all O3∶K6.The results of antibiotic sensitivity analysis showed that these strains were only resistant or moderately sensitive to cefazolin(CFZ),and sensitive to other antibiotics.The PCR results of showed that tdh+and trh-strains were the main virulence genes.The DNA fingerprints after restriction endonuclease(Not I)digestion showed that the similarity value among the strains was 68.3%~100.0%,which could be divided into 21 types according to 100%similarity.Conclusions There was a high degree of homology among strains of two foodborne diseases at different time and different places.DNA fingerprints of strains with different serotypes were closely related,which indicated that there might be a continuous pollution of Vibrio parahaemolyticus and with variation in molecular level.
