摘　　要：目的探讨机器人辅助腹腔镜下前列腺癌根治术的疗效及其对护理的影响。方法比较2018年12月-2019年12月期间在复旦大学附属华东医院进行治疗的20例达芬奇机器人辅助腹腔镜下前列腺癌根治术患者(RALP组)与20例行腹腔镜下前列腺癌根治术患者(LRP组)的手术时间、围术期输血率、引流管拔除时间、术后住院天数、术后尿控率及前列腺特异抗原等指标,以及两种手术对护理的影响,进行比较和统计学分析。结果RALP组的手术时间(210.0±51.0)min短于LRP组(380.0±94.0)min;RALP组输血率5%,小于LRP组的30%,RALP组引流管拔除时间(4.8±0.8)d短于LRP组(7.2±1.2)d,RALP组术后住院天数(7.8±0.8)d短于LRP组(9.6±1.1)d,RALP组术后6月尿控率85%,优于LRP组的60%,以上各参数两组间的差异有统计学意义。结论与常规腹腔镜手术比较,机器人辅助腹腔镜下前列腺癌根治术可减少手术时间,减少输血率,尿控恢复快,促进患者术后早期恢复,能减轻术后护理工作量,提高护理质量。Objective To explore the curative effects of robot-assisted laparoscopic radical prostatectomy and its influence on nursing.Methods 20 patients with prostate cancer who underwent Da Vinci robot-assisted laparoscopic radical prostatectomy in the Huadong Hospital Affiliated to Fudan University from December 2018 to December 2019 were selected as RALP group,and 20 ones who underwent conventional laparoscopic radical prostatectomy at the same time were selected as LRP group.The operation time,perioperative blood transfusion rate,drainage tube removal time,postoperative hospital stay,postoperative urinary control rate and prostate-specific antigen level and other indexes were compared between the two groups,as well as the impact of two kinds of surgery on nursing care.Results The operation time of RALP group(210.0±51.0)minutes was shorter than that of LRP group(380.0±94.0)minutes.The blood transfusion rate in RALP group(5%)was less than that in LRP group(30%).The drainage tube removal time of RALP group(4.8±0.8)days was shorter than that of LRP group(7.2±1.2)days.The postoperative hospital day of RALP group(7.8±0.8)days was shorter than that of the LRP group(9.6±1.1)days.6 months after the operation,the urinary control rate of RALP group(85%)was higher than that of LRP group(60%).The differences in the above parameters between the two groups were statistically significant.Conclusion Compared with conventional laparoscopic surgery,robot-assisted laparoscopic radical prostatectomy can shorten the operation time,reduce the blood transfusion rate,promote the early recovery of the patient after the operation,and the recovery of urinary control is faster.At the same time,it can reduce the workload of postoperative care and improve nursing care quality.
