摘  要:目的了解滕州市居民膳食及生活行为方式对食管癌发病的影响,为制定防治策略提供依据。方法选取2017~2019年省级医院及滕州市中心人民医院经胃镜、临床病理检查确诊的新发食管癌住院患者作为病例组,在同期住院患者中选取对照组,按照病例与对照1∶1配对设计进行居民膳食及生活行为方式对食管癌发病影响的条件Logistic回归分析。结果病例组、对照组各106例,病例组平均年龄为(60.86±9.36)岁,对照组为(60.20±9.01)岁,男性72对,女性34对,男女之比为2.12∶1,有吸烟、饮酒、新鲜蔬菜、腌制食品、热烫饮食、干硬食物、进食快、三餐不规律、经常生闷气和肿瘤家族史10个因素进入条件Logistic回归模型,OR值分别为2.423、3.525、0.423、2.081、2.947、3.125、2.756、1.963、1.814和2.847,即吸烟、饮酒、经常进食腌制食品、热烫饮食、干硬食物、进食快、三餐不规律、生闷气和有肿瘤家族史是EC发生的影响因素,多食新鲜蔬菜可降低EC发生风险(P<0.05或<0.01)。结论膳食结构和饮食习惯是导致EC发病可改变的因素,居民减少对食管"辛辣、热烫、快、咸、硬"等物理刺激,可有效降低EC发病风险。Objective To understand the influence of diet and lifestyle on the incidence of esophageal cancer among residents in Tengzhou city, so as to provide the basis for formulating prevention and treatment strategies. Methods The patients who were newly diagnosed esophageal cancer by gastroscope and clinical pathology were collected from Shandong provincial hospital and Tengzhou central people’s hospital were selected as case group, and the inpatients in the same period were selected as control group according to the 1:1 matched design, the conditional logistic regression analysis was used to analyze the influence of diet and lifestyle on the incidence of esophageal cancer. Results There were 106 cases in case group and 106 cases in control group.The average age of the case group and control group were(60.86±9.36) and(60.20±9.01) years old.There were 72 pairs of males and 34 pairs of females, with the ratio of male to female of 2.12∶1.There were 10 factors including smoking, drinking, fresh vegetables, pickled food, hot and hot diet, dry and hard food, fast eating, irregular meals, frequent sulking and family tumor history were included in the conditional logistic regression model for analysis, and the OR values were 2.423,3.525,0.423,2.081,2.947,3.125,2.756,1.963,1.814 and 2.847,respectively.Smoking, drinking, regular consumption of preserved food, hot and hot diet, dry and hard food, fast eating, irregular meals, sulking and family history of tumor were the influencing factors for the occurrence of esophageal cancer and eating more fresh vegetables can reduce the risk of esophageal cancer. Conclusion Dietary structure and eating habits are the factors that can change the incidence of esophageal cancer.Educating residents to reduce physical stimuli such as "spicy, hot, fast, salty and hard" of esophagus can effectively reduce the risk of EC.
