摘 要:目的探索颞骨解剖训练在耳鼻咽喉科住院医师耳内镜手术培训教学中的应用及效果。方法选取2016年1月及2019年12月在中山大学附属第一医院耳鼻咽喉科进行耳内镜手术培训62名住院医师为研究对象,按参加耳科规培时间先后分为对照组和试验组,2016年1月-2017年12月期间30名住院医师为对照组,2018年1月-2019年12月期间32名住院医师为试验组,两组住院医师在耳科病区参加耳科患者管理、耳内镜手术;试验组教学方式增加耳内镜颞骨解剖训练。耳科培训结束,两组均进行专业理论考试、手术操作考核及问卷调查,对比两种教学方式在耳内镜手术教学效果。结果两组住院医师在年龄、性别、学历、培训前耳科专业理论成绩等方面的差异无统计学意义(均P>0.05)。参加耳内镜颞骨解剖培训后,试验组医师对耳内镜下中内耳解剖结构辨认能力明显提高。耳科规培结束,耳科专业理论及耳内镜手术操作成绩,试验组优于对照组,差异具有统计学意义(P<0.05)。试验组医师在学习兴趣、中内耳结构辨认能力、耳内镜手术熟练程度、手术风险预测能力、术中应变能力、医患沟通能力、对教学方式满意率等方面评价优于对照组,差异均具有统计学意义(P<0.05)。结论颞骨解剖训练在耳内镜手术培训教学中具有一定的优势,可有效提高耳内镜手术培训教学质量。Objective To explore the application and effect of temporal bone anatomy training in otolaryngology residents’otoendoscopic surgery training.Methods 62 residents who received otoendoscopic surgery training in the Department of Otorhinolaryngology,the First Affiliated Hospital of Sun Yat-sen University in January 2016 and December 2019 were selected and divided into the control group and the experimental group according to the training time.30 residents from January 2016 to December 2017 were assorted into the control group,and 32 residents from January 2018 to December 2019 were selected as the experimental group.the experimental group was given otoendoscopy training.At the end of otology training,professional theory examination,surgical operation examination and questionnaire survey were conducted in both groups to compare the effect of the two teaching methods in otoendoscopy surgery teaching.Results There was no significant difference in age,gender,education background and theoretical scores of otology before training between the two groups(P>0.05).After the training of otoendoscopy and temporal bone anatomy,the ability to identify the anatomical structure of the middle and inner ear was significantly improved in the experimental group.The experimental group performed better than the control group in learning interest,middle and inner ear structure identification ability,ear endoscopic surgery proficiency,surgical risk prediction ability,intraoperative strain ability,doctor-patient communication ability,satisfaction rate of teaching methods and other aspects(P<0.05 each).Conclusion Temporal bone anatomy training has certain advantages in the teaching of otoendoscopic surgery,and can effectively improve the teaching quality of otoendoscopic surgery training.
