摘 要:目的探讨在妇产科住院医师规范化培训(简称住培)临床实践技能培训中引入翻转课堂方案的应用模式及可行性。方法选择南通大学附属医院、南通市妇幼保健院、南通市第一人民医院2017~2019级60名妇产科住院医师作为研究对象,随机单盲分为两组。对照组(30名)采用常规带教师资为主,住院医师带教为辅的指导学习模式;观察组(30名)采用住院医师为主体的翻转课堂学习模式,包括住院医师与住培带教师资角色翻转、住院医师与患者角色翻转、住院医师与考官角色翻转。对两组住院医师在临床病史采集及书写规范、床位管理满意度、教学查房、医护合作配合、妇产科常规基本手术操作规范性等妇产科实践操作技能培训中医护合作满意度、住院医师对教学满意度、患者对住院医师满意度,以及住院医师结业考核成绩进行对比。结果观察组住院医师在医护合作满意度、住院医师对教学满意度、患者对住院医师满意度方面均显著高于对照组(P<0.05);观察组住院医师结业成绩(92.1±1.7)分显著高于对照组住院医师结业成绩(80.0±2.0)分,差异有统计学意义(P<0.05)。主要表现为观察组住院医师的自信心明显增加;责任感明显增强;建立了良好医患、医护关系,能够更多体现医学人文关怀;有利于理论与实践的高效结合,并能在实践中发现更多的临床科研问题,促进实践操作流程的规范及创新。结论妇产科住培临床实践技能培训中翻转课堂的应用,可明显提高住院医师、医患、医护满意度及考核成绩,最终促进妇产科住院医师综合能力的提升,值得推广。Objective To explore the application mode and feasibility of the flipped classroom in the standardized training of obstetrics and gynecology residents.Methods Sixty obstetrics and gynecology residents in Nantong University Affiliated Hospital,Nantong Maternal and Child Health Hospital,and Nantong First People's Hospital from 2017 to 2019 were selected as the experimental subjects.The single blind grouping method was applied:residents in the control group(n=30)received regular tutor-centered,guidance learning mode;in the experimental group(n=30)residents were the main body of the flipped classroom,including the role reversal of residents and trainers,residents and patients,and residents and examiners.The two groups were mainly trained in clinical history collection and writing norms,bed management,teaching rounds,medical and nursing schedules,diagnosis and treatment cooperation,and standardization of routine basic operations in obstetrics and gynecology.The satisfaction of medical and nursing cooperation,residents satisfaction with teaching,patient's satisfaction with residents,and the results of graduation examination were compared between the two groups.Results In the experimental group residents'satisfaction with medical cooperation and teaching,and patients'satisfaction with students were significantly higher than those in the control group(P<0.05).The assessment scores of residents in the experimental group(92.1±1.7)were significantly higher than those in the control group(80.0±2.0),with statistical significance(P<0.05).The main demonstrations were that residents in the experimental group showed significant increase in self-confidence and sense of responsibility;established better doctor-patient and medical care relationships,reflecting more medical humanistic care;combined theory and practice more efficiently and found more clinical research problems in practice,promoting the standardization and innovation of practical process.Conclusion The application of the flipped classroom in the standardized training
