摘 要:目的研究计算机辅助诊断(CAD)系统在甲状腺单发结节和多发结节不同情况下的诊断价值。方法前瞻性纳入甲状腺患者279例(共515个甲状腺结节),分为单结节组和多结节组。分析比较CAD和各级不同水平超声医师对于两组病例的诊断效能。结果CAD诊断甲状腺单结节病例的敏感性与高级超声医师相当(88.52%vs.89.41%,P>0.05),诊断多结节组的敏感性与中级超声医师相当(81.61%vs.82.39%,P>0.05),CAD对于单结节病例组的诊断效能优于多结节病例组;CAD诊断的敏感性较高,但特异度低于超声医师。超声医师自身比较,对于单结节组和多结节组其诊断效能无明显差异。结论计算机辅助诊断具有良好的临床应用价值,诊断特异度偏低,对于甲状腺多发结节的病例超声医师审核至关重要。Objective To evaluate the diagnostic value of computer aided diagnosis(CAD)system in different cases with single and multiple thyroid nodules.Methods A total of 279 patients with thyroid nodules(a total of 515 thyroid nodules)were prospectively enrolled,and they were divided into the single-nodule group and the multi-nodule group.We compared and analyzed the diagnostic efficiency of CAD and that of different levels of sonographers for the cases in both groups.Results CAD had sensitivity similar to that of senior sonographers in the diagnosis of thyroid nodules(88.52%vs.89.41%,P>0.05),and had sensitivity similar to that of intermediate sonographers in the diagnosis of multiple nodules(81.61%vs.82.39%,P>0.05).The diagnostic efficiency of CAD for single nodule was better than that for multiple nodules.The sensitivity of CAD diagnosis was high,but its specificity was lower than that of sonographers.For sonographers’dignosis,there was no significant difference in the diagnostic efficacy between the single-nodule group and the multi-nodule group.Conclusion CAD has good clinical application value with low diagnostic specificity.It is of great importance for sonographer’s review of cases with multiple thyroid nodules.
