摘  要:目的探讨以临床胜任力为导向的以问题为基础(PBL)教学在八年制外科见习带教中的应用效果。方法研究对象选取2018年8月—2019年12月期间就读临床八年制的58名医学学生。以临床胜任力为导向的PBL教学模式将2015年级临床八年制30名学生分为研究组,采取传统教学模式选取2014级临床八年制共28名学生分为对照组,通过自制调查表及评分表,比较两组教学效果,两组外科基本操作技能与教学满意度。结果两组教案的编写评分、增强知识掌握能力差异无统计学意义(P>0.05),除此之外,研究组增强合作能力、增强发散性思维能力、提高聆听、问诊技能、增强自主学习、查阅文献能力等8项评分均显著高于对照组(P<0.05);研究组学生理论出科考核成绩及穿脱术衣、手套成绩稍高于对照组(P>0.05),见习结束时,研究组学生消毒铺巾,缝合打结,伤口换药,止血包扎成绩均高于对照组(P<0.05);与对照组学生总满意度82.14%相比,研究组总满意度100%明显较高(P<0.05)。结论在八年制外科见习带教中采取以临床胜任力为导向的PBL教学有利于提高学生外科基本操作技能,提升学生临床胜任力,保证教学效果,保证学生对教学的满意度。Objective To explore the application effect of problem-based teaching(PBL)guided by clinical competence in the eight-year Surgical Probation Teaching.Methods 58 medical students enrolled in the eight-year clinical program during August 2018 to December 2019 were selected as study subjects.The clinical competence-oriented PBL teaching mode divided 30 students from the clinical eight-year program of grade 2015 into study group,and the traditional teaching mode selected 28 students from the clinical eight-year program of grade 2014 into control group.The teaching effect,basic surgical operation skills and teaching satisfaction of the two groups were compared through self-made questionnaire and scoring table.Results There was no significant difference between the two groups(P>0.05).In addition,the eight scores of the study group were significantly higher than those of the control group(P<0.05).The results of academic examination,dressing and gloves in the study group were slightly higher than those in the control group(P>0.05).At the end of the internship,the students in the study group achieved better results in disinfection,bandaging,suture,dressing change and hemostasis than those in the control group(P<0.05).Compared with 82.14%in the control group,100%in the study group was significantly higher(P<0.05).Conclusion In the eight-year surgical internship teaching,PBL teaching guided by clinical competence is conducive to improving students'basic surgical operation skills,improving students'clinical competence,ensuring the teaching effect and ensuring students'satisfaction with teaching.
