摘 要:目的探讨口腔种植机器人在口腔种植手术中应用的有效性和安全性。方法2020年9~10月对我科66例牙列缺损(81颗牙齿缺失)拟行种植手术者术前均行口腔锥形束CT(cone beam CT,CBCT)检查,将其影像数据输入种植机器人导航定位系统进行三维重建并行术前种植体三维位置规划。局麻消毒后行患者及机械臂的注册,导航下全程机械臂完成种植窝洞的制备和种植体植入,术后CBCT检查验证种植体误差。结果口腔种植机器人术前准备时间(描述手术注册到机械臂开始实施窝洞制备的时间)3~23 min,平均8 min。所有种植手术均顺利完成,术中未发生并发症。种植体植入点总误差为(0.63±0.23)mm,根尖点总误差为(0.64±0.26)mm,角度误差为2.27°±0.98°。结论口腔种植机器人进行口腔种植手术定位精度高,可根据缺牙区牙槽骨的形态进行术前手术规划,避免周围重要神经及解剖结构的损伤,临床效果满意。Objective To evaluate the efficacy and safety of dental implant robot Remedent for dental implant operation.Methods A total of 66 patients with 81 teeth missing were admitted to our department for dental implant operation from September 2020 to October 2020.All the patients underwent cone beam CT(CBCT)scan before surgery.The CBCT DICOM data were uploaded into the robotic planning software for 3D reconstruction,and virtual preoperative three-dimensional implant position planning was carried out.After local anesthesia and disinfection,the patient and the robotic arm were registered,and the implant fossa and cavity were prepared and implanted with the robotic arm under navigation.Postoperative CBCT was taken to evaluate the deviations between the virtual and the actually placed implants.Results The preoperative preparation time of the dental implant robot Remedent(describing the time of the operation registration to the robotic arm for the preparation of the cavity)was 3-23 min,with an average of 8 min.All the implant operations were successfully completed without complications.The total error of the implant point was(0.63±0.23)mm,the total error of the apical point was(0.64±0.26)mm,and the angle error was 2.27°±0.98°.Conclusions The dental implant robot Remedent has high positional accuracy.Preoperative surgical planning can be carried out according to the shape of alveolar bone in the missing area,so as to avoid the injury of important peripheral nerves and anatomical structures,and achieve satisfactory clinical effect.
