摘 要:目的建立食物中毒病原菌的快速检测方法,并进行分子溯源和耐药分析。方法使用多重PCR、国标方法及BAX Q7全自动病原菌检测系统,对新乡市某托辅中心食物中毒可疑标本进行致病菌检测,采用脉冲场凝胶电泳分子分型试验和BioNumerics Version软件进行溯源分析,使用全自动药敏接种判读仪(AIM-Vizion)进行耐药分析。结果采样后24 h内即确定食物中毒病原菌,共计从患者粪便和砧板中分离出9株志贺氏Ⅰ相菌,所有菌株同源性100%。分离菌株对氨苄西林、四环素、左氟沙星、强力霉素、氨苄西林/舒巴坦、萘啶酸、磺胺类药物、米诺沙星、链霉素、氨曲南、氯霉素、阿奇霉素和环丙沙星全部耐药,对头孢吡肟、左氟沙星、氯霉素、阿莫西林/克拉维酸、头孢他啶、米诺环素的耐药率为55.6%~88.9%,对亚胺培南和美罗培南高度敏感。结论国标方法结合多重PCR和BAX Q7系统在食物中毒检测中快速有效,此次食物中毒由同一来源的宋内志贺Ⅰ相菌引起,且分离菌株多重耐药情况严重,需合理选择临床药物,并加强全市细菌性痢疾监测。Objective To establish a rapid method detecting Shigella sonnei,a pathogenic bacteria causing food poisoning and to further study the molecular typing and drug resistance of the pathogen.Methods The suspected food-poisoning samples from a after-school care center were collected and analyzed by multiple PCR,national testing standard methods and pathogenic detection system(BAX Q7).PFGE and BioNumerics software were used for molecular typing of the identified isolates.Drug resistance analysis was carried out by Vizion.Results The pathogen of the event was quickly detemined in 24 h after sampling,and 9 strains of Shigella sonnei phaseⅠwere isolated from the chopping board and faecal specimens of the patients.PFGE genotyping indicated that all isolates in this outbreak showed 100%homology.The isolates were all resistant to ampicillin,tetracycline,levofloxacin,doxycycline,ampicillin/sulbactan,naphthoxine,sulfonamides,minoxacin,streptomycin,amtramine,chloramphenicol,azithroxacin and ciprofloxacin,and the resistance rate of cefepime,levofloxacin,chloramphenicol,amoxicillin/clavulanic acid,ceftazidime and minocycline ranged from 55.6%~88.9%,while no resistance was found to imipenem and meropenem.Conclusion National standard method,together with multiple PCR and BAX Q7 system,is rapid and effective in food poisoning testing.This event was caused by Shigella sonnei Phase I which came from the same source,and multiple drug resistance of the isolates was serious.It is necessary to select the clinical medicine reasonably and to strengthen bacterial dysentery surveillance in Xinxiang.
