摘 要:目的探讨在外科住院医师规范化培训中应用情景模拟结合案例教学法进行医患沟通能力培养的效果。方法研究对象是南方医科大学附属东莞医院外科系统规培的48名住培医师和10名带教老师,研究时间为2018年1月—2019年12月,带教老师采取情景模拟结合案例教学的方法对外科住培医师进行医患沟通能力的培训;评价方法:(1)采用医患沟通技能评价量表(SEGUE量表),比较学员接受情景模拟结合案例教学法前后的医患沟通能力评分;(2)记录学员对其所接受的医患沟通教学方法的满意度问卷调查。结果外科规培学员在整体医患沟通能力评分和信息采集、信息给予和理解病人三个维度的评分培训后明显优于培训前,差异具有统计学意义(P<0.05);外科住培医师对培训的问卷调查提示,在提高临床沟通意识、掌握医患沟通的基本知识、掌握医患沟通的技巧、专业知识的应用、提高对患者的人文关怀意识和适应临床医生工作六个方面均有一定的帮助。结论情景模拟结合案例教学法的培训模式有利于提高外科住培医师的医患沟通能力,值得推广。Objective To explore the effect of applying Scenario Simulation Teaching and case-based learning in the standardized training of surgical residents.Methods The subjects of the study were 48 resident students and 10 teachers in the Surgical System of Dongguan Hospital affiliated to Southern Medical University.The study runs from January 2018 to December 2019.The teacher adopts the method of Scenario Simulation Teaching and case-based learning to train the students in communication ability between doctors and patients.Evaluation methods:(1)using the doctor-patient communication skills evaluation scale(SEGUE scale)to compare the students'acceptance situation simulation combined with the case teaching method before and after the doctor-patient communication ability score;(2)to record the students'satisfaction with the doctor-patient communication teaching method they accepted.Results Surgical training students were better than pre-training students in the three dimensions of overall doctor-patient communication ability score and information collection,information giving and understanding of patients,the difference was statistically significant,(P<0.05).The questionnaire survey showed that it is helpful to improve the consciousness of clinical communication,master the basic knowledge of doctor-patient communication,master the skill of doctor-patient communication,apply professional knowledge,improve the humanistic care consciousness of patients and adapt to the work of clinicians.Conclusion The training mode of situational simulation combined with case teaching method is helpful to improve the communication ability of doctors and patients,which is worth popularizing.
