摘 要:目的探讨复方α-酮酸对尿毒症透析患者氨基酸谱及免疫功能的影响。方法血透>3个月尿毒症患者240例,随机分成2组。观察组180例予低蛋白饮食联合口服复方a酮酸,对照组60例仅低蛋白饮食,治疗24周。比较治疗前后氨基酸及CD3^(+)CD4^(+)CD8^(+)T淋巴细胞水平百分比变化。结果治疗前两组患者,血清肌肝、白蛋白、EMI指数、游离氨基酸、CD3^(+)、CD4^(+)、CD8^(+)T淋巴细胞水平差异无统计学意义。治疗后,观察组比治疗前及对照组治疗后,除苯丙氨酸(72.13±26.60)μmol/L外,其余七种必需氨基酸均升高(P<0.05);治疗后,观家组及对照组CD3^(+)T淋巴细胞、CD3^(+)CD4^(+)T淋巴细胞百分比较治疗前升高(P<0.05)。CD3^(+)CD8^(+)T淋巴细胞百分比无变化。结论长期透析患者口服复方α-酮酸片可改善氨基酸谱及免疫功能。Objective To investigate the effect of oral compound alpha ketonic acid on amino acid profile and immune function in patients undergoing long-term dialysis.Methods 240 uremic patients with hemodialysis for more than 3 months were randomly divided into two groups.180 cases in the observation group were given low protein diet combined with oral compound alpha ketonic acid,while 60 cases in the control group were given low protein diet only for 24 weeks.The changes of blood amino acid and percentage of CD3^(+),CD4^(+),CD8^(+)T lymphocyte were compared before and after treatment.Result After treatment,there was no significant difference between the observation group and the control group(P>0.05),except phenylalanine,the other seven essential amino acids increased significantly(P<0.05),the percentage of CD3^(+),CD4^(+)T lymphocyte subsets increased(P<0.05),and the percentage of CD8^(+)T lymphocyte level had no statistical significance(P<0.05).Conclusion Oral administration of compound alpha-ketoacid tablets can improve amino acid profiles and immune fiinction in long-tenn dialysis patients.
