成人抑郁症患者脑结构异常的危险因素分析

摘　　要：目的分析成人抑郁症患者脑结构异常的相关危险因素。方法回顾性分析2017年7月1日至2018年12月31日首都医科大学附属北京安定医院收治的466例抑郁症患者的临床资料及头颅磁共振成像(MRI)资料,依据头颅MRI结构是否异常分为脑结构异常组(250例)和脑结构正常组(216例),比较两组人口学及临床特征差异,探讨成人抑郁症患者脑结构异常的相关危险因素。结果脑结构异常组年龄大于脑结构正常组[(44±13)岁比(38±13)岁](P<0.01),有精神病性症状、自杀观念的比例高于脑结构正常组[38.8%(97/250)比26.4%(57/216)、44.8%(112/250)比29.2%(63/216)](P<0.01)。二分类Logistic回归分析结果显示,年龄(OR=1.034,95%CI 1.020~1.049)、伴有精神病性症状(OR=2.119,95%CI 1.396~3.218)、自杀观念(OR=2.096,95%CI 1.403~3.131)是成人抑郁症患者脑结构异常的危险因素(均P<0.01)。结论年龄大、存在自杀观念以及伴有精神病性症状可能是成人抑郁症脑结构异常的危险因素。Objective To analyze the risk factors of structural abnormalities of brain in adult patients with depression.Methods A retrospective analysis of the brain magnetic resonance imaging(MRI)data of 466 inpatients with depression admitted to Beijing Anding Hospital,Capital Medical University from Jul.1,2017 to Dec.31,2018 was done,and the cases were divided into an abnormal brain structure group(250 cases)and a normal brain structure group(216 cases)based on the presence or absence of abnormalities in the brain structure.The demographic and clinical differences between the two groups were compared to explore the risk factors of brain structure abnormalities in the adult patients with depression.Results The age of the abnormal brain structure group was higher than that of the normal brain structure group[(44±13)years old vs(38±13)years old](P<0.01),the proportions of psychotic symptoms and suicidal thoughts were higher than those of the normal brain structure group[38.8%(97/250)vs 26.4%(57/216),44.8%(112/250)vs 29.2%(63/216)](P<0.01).Binary Logistic regression analysis showed that age(OR=1.034,95%CI 1.020-1.049),accompaniment by psychotic symptoms(OR=2.119,95%CI 1.396-3.218),suicidal idea(OR=2.096,95%CI 1.403-3.131)were risk factors for abnormal brain structure in the adult patients with depression(all P<0.01).Conclusion The old age,accompaniment by psychotic symptoms and the existence of suicide idea may be risk factors for abnormal brain structure in adults with depression.
