摘　　要：目的探讨趾背单边横位皮瓣治疗4、5趾并趾伴小趾多趾畸形的临床疗效。方法自2014年7月至2019年2月我院收治4、5趾并趾伴小趾多趾畸形患儿13例,4、5趾均为软组织并连。手术切除多趾,关节面成形或楔形截骨治疗小趾外侧偏斜,趾背单边横位皮瓣重建趾蹼。术后根据足趾功能,趾蹼外观、深度、坡度以及主观满意度评价手术效果。结果13例患儿随访6个月至2年,平均13个月。重建后趾蹼的外观、颜色、弹性、质地与健侧趾蹼近似,小趾外侧偏斜完全矫正。随访6个月时,术后趾蹼深度、坡度、足趾外展度与健侧进行比较,差异无统计学意义(P>0.05)。结论趾背单边横位皮瓣用于4、5趾并趾伴小趾多趾畸形的治疗,术中无需植皮,手术操作相对简便,术后皮瓣成活率高,重建的趾蹼外观及足趾功能与健侧足趾近似。Objective To explore the clinical efficacy of the dorsal toe transverse flap in the treatment of the correction of the fourth and fifith toe polydactyly and syndactyly.Methods From July 2014 to February 2019,13 children with the fourth and fifith toe syndactyly combined with small toe polydactyly were treated in our hospital.The fourth and fifth toe were soft tissue combined.The lateral deviation of the small toe was treated by polydactyly excision,arthroplasty or cuneiform osteotomy,and the unilateral transverse flap was used to reconstruct the web of the toe.The results were evaluated according to toe function,toe web appearance,depth,slope,and subjective satisfaction.Results All the patients were follow-up for 6 months to 2 years,with an average of 13 months.After reconstruction,the appearance,color,elasticity,and texture of the toe web were similar to those of the healthy side.The lateral deviation of the small toe was completely corrected.After 6 months of follow-up,there was no significant difference in toe web depth,slope,and abduction between the two sides(P>0.05).Conclusion The unilateral transverse flap was used for the treatment of the fourth and fifith toe syndactyly combined with small toe polydactyly.There was no need for skin grafting during the operation.The operation was relatively simple,the survival rate of the flap was high,and the reconstructed toe web appearance and toe function were similar to that of the healthy side.
