摘 要:目的探讨程序性细胞死亡配体1(programmed cell death ligand 1,PD-L1)及M2型肿瘤相关巨噬细胞(tumor-associated macrophages,TAMs)在胃腺癌组织中的表达及预后意义。方法收集2011年5~12月苏州大学附属第一医院存档的胃腺癌手术切除标本268例,采用免疫组化EnVision法检测胃腺癌组织中PD-L1及CD163(M2型TAMs特异性标志物)蛋白表达,并分析两者表达与胃腺癌临床病理特征及预后的相关性。结果胃腺癌癌细胞中PD-L1的高表达率为19.4%(52/268);胃腺癌间质细胞中PD-L1的高表达率为62.3%(167/268)。PD-L1表达与CD163表达呈正相关。Kaplan-Meier生存曲线分析表明,胃腺癌间质细胞中PD-L1表达与患者预后相关(P<0.05),CD163表达与患者生存无关,在对患者进行亚组分析时发现PD-L1高表达同时CD163低表达的患者预后最好;PD-L1低表达同时CD163高表达的患者预后最差。结论PD-L1在胃腺癌组织中的表达与CD163表达呈正相关,提示TAMs可作为胃癌联合治疗的新潜在靶点,并提高PD-1/PD-L1免疫卡控点治疗胃癌的有效率。Purpose To investigate the expression of programmed cell death ligand 1(PD-L1)and M2-like tumor-associated macrophages in gastric adenocarcinoma as well as the relationship with patients prognosis.Methods A total of 268 patients from May 2011 to December 2011 in the First Affiliated Hospital of Soochow University were collected.We evaluated the protein expression of PD-L1 and CD163(M2-like tumor-associated macrophages specific marker)in tumor tissues using immunohistochemistry(IHC)staining,as well as analyzed the correlation between clinicopathological features and patients prognosis.Results Patients with high expression of PD-L1 on tumor cells accounted for 19.4%(52/268),while high expression of PD-L1 on interstitial cells accounted for 62.3%(167/268),PD-L1 expression was positively associated with CD163 expression.The Kaplan-Meier analysis demonstrated that PD-L1 expression was significantly associated with better prognosis(P<0.05),while no correlation was observed between CD163 expression and patients survival time.Subgroup analysis demonstrated that high expression of PD-L1 in combination with low expression of CD163 were significantly associated with better prognosis,whereas low expression of PD-L1 with high expression of CD163 indicated worse prognosis.Conclusion The PD-L1 expression is positively associated with CD163 in gastric adenocarcinoma tissues,suggesting that tumor-associated macrophages may serve as a new potential target combinated therapy was of gastric cancer and improve the efficiency rate of PD-1/PD-L1 immune checkpoint in the treatment of gastric cancer.
