眼科住院医师规范化培训结业临床实践能力统一考核的探索与实践

摘　　要：目的分析首次广东省眼科住院医师规范化培训(简称住培)结业技能统一考核实践,通过总结问题与经验推进全面客观及同质化的能力评估,促进眼科住培质量的不断提升。方法系统回顾该次眼科住培结业技能统一考核的实施情况,对评价体系、考试设计、考务组织与考核实施等进行详细分析,统计并比较不同考区、不同站点的考生成绩。结果该次统考改变了以往由各基地自行组织结业考核的形式,针对不同身份背景、不同住培基地、不同培训过程的学员,圆满高效地实施了全省统一的考核方案、考试组织以及评分要求。考生成绩符合正态分布,Cronbach系数为0.452,各个站点的难度系数的区间为0.700,0.869,各考站平均成绩最低分为阅片站69.57分,最高分为问诊检查站86.90分。结论该次广东省眼科住培结业技能统考难度与区分度适宜,较好地实现了考试的规范同质以及公平公正,也比较客观反映了各住培基地的培训成果,在考务信息化建设、标准化病人、考生评价等方面有待进一步完善。Objective To Analyze the first unified skills tests of ophthalmology residents in Guangdong province,this paper summarizes the problems and experiences,which aims to promote the comprehensive,objective and homogeneous assessment of residents'capability and continuous improvement of the quality.Methods This research reviewed the practice of this unified tests and analyzed the assessment system,examination design,organization and implementation.The assessment results among different examination sites were also counted and compared.Results This unified examination has changed the previous form of completion examination organized by the base hospitals themselves.For residents with different identity backgrounds,training bases,and training processes,the unified assessment program was implemented successfully and efficiently.The result of the examination conforms to the normal distribution,the Cronbach coefficient of the score is 0.452.The difficulty coefficients of each site ranged from 0.700 to 0.869,average score was 69.57 at the image-reading station and 86.90 at the consultation station.Conclusion The difficulty and distinction of this unified skills tests of ophthalmology is suitable,which demonstrates the standardization,homogeneity and fairness of the assessment,also reflects the training results of each base hospital objectively.In the future,the information construction,standardized patients,examinee evaluation and other aspects need to be further improved.
