摘　　要：目的探讨行机器人辅助腹腔镜肾部分切除术(RAPN)治疗巨大肾血管平滑肌脂肪瘤(≥7 cm)的安全性及有效性。方法回顾性分析南昌大学第一附属医院2016年至2019年41例行RAPN治疗巨大肾血管平滑肌脂肪瘤的患者资料。结果41例患者手术均顺利完成,其中1例术中输血并中转开放行肾部分切除术,所有患者术后均未出现并发症。手术时间175(145~245)min,热缺血时间(21.0±3.2)min,估计出血量200(100~225)mL,术后住院时间7(6~8.5)d。术后病理均为肾血管平滑肌脂肪瘤。41例随访(23.5±4.8)个月,术后6个月肾小球滤过率(72.8±6.3)mL/min,随访期内均无复发。结论RAPN治疗巨大肾血管平滑肌脂肪瘤安全有效,患者肾功能保留良好。Objective To investigate the safety and efficacy of robot-assisted partial nephrectomy(RAPN)for giant renal angiomyolipoma(diameter≥7 cm).Methods Clinical data of 41 patients treated in our hospital during 2016 to 2019 were retrospectively analyzed.Results All operations were successful.One patient received intraoperative blood transfusion and converted to open partial nephrectomy.No complications were observed.The operation time was 175(145-245)min,hot ischemia time was(21.0±3.2)min,estimated blood loss was 200(100-225)mL,and length of postoperative hospital stay was 7(6-8.5)d.The pathological diagnosis of all patients was renal angiomyolipoma.During the follow-up of(23.5±4.8)months,the 6-month glomerular filtration rate was(72.8±6.3)mL/min,and no recurrence occurred.Conclusion RAPN is safe and effective in the treatment of giant renal angiomyolipoma.Patients renal function is well preserved.
