摘  要:目的探讨病案教学法结合迷你临床演练评估测评在急诊全科规范化培训中的教学效果及学生满意度,以提高对于全科规培医师的急诊教学水平。方法以2016年10月1日-2020年10月30日在某院急救医学中心进行3年住院医师培训的24名全科规培医师为研究对象,随机分为观察组和对照组,每组12名学员,观察组采用病案教学法,对照组采用传统教学法,以Mini-CEX测评考核联合教学满意度问卷调查,比较2组教学效果及学员满意度。结果两组教学效果比较,观察组的医患交流(7.53±1.22)分、体格检查(7.48±1.47)分、疾病诊断(7.64±1.03)分、沟通技巧(7.18±1.65)分、人文关怀(7.88±0.54)分、组织效能(7.05±1.17)分、整体表现(7.57±1.38)分均高于对照组(6.70±1.13、6.28±1.32、6.42±1.63、6.27±1.71、7.21±0.88、6.21±1.91、6.34±1.47)分,差异具有统计学意义,P<0.05。学生对教学满意度调查观察组满意率为100%高于对照组满意度75%,差异具有统计学意义,P<0.05。结论病案教学法及Mini-CEX测评结合兼具教学与考核的双重作用,既能提高全科规培学员临床实践能力,增强学员满意度,提升学习效率;又可以客观评价教学效果,便于指导教师总结经验及优化教学流程,适合急诊教学应用。Objective To explore the effect of Case-based Teaching Method combined with mini-CEX evaluation in standardized training teaching of emergency and the satisfaction of students,in order to improve the level of emergency teaching.Methods 24 general practitioners who received 3-year resident training in the emergency medical center of our hospital from October 1,2016 to October 30,2020 were randomly divided into observation group and control group,with 12 traineesineachgroup.The observation group adoptedCase-based teaching method,while the control group adopted traditional teaching method.The teaching effect and satisfaction of students of the two groups was evaluated by mini-CEX evaluation and teaching satisfaction questionnaire.Results Comparison of teaching effects between the two groups.The scores of doctor-patient communication(7.53±1.22),physical examination(7.48±1.47),disease diagnosis(7.64±1.03),communication skills(7.18±1.65),humanistic care(7.88±0.54),organizational effectiveness(7.05±1.17)and overall performance(7.57±1.38)in the observation group were higher than those in the control group(6.70±1.13,6.28±1.32,6.42±1.63,6.27±1.71,7.21±0.88,6.21±1.91,6.34±1.47).The difference was statistically significant(P<0.05).In the survey of students’satisfaction with teaching,the satisfaction rate of the observation group was 100%higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of medical record teaching method and mini CEX evaluation has the dual functions of teaching and assessment,which can not only improve the clinical practice ability of general practitioners,enhance their satisfaction and improve their learning efficiency.It can also objectively evaluate the teaching effect,which is convenient for teachers to summarize experience and optimize the teaching process,and is suitable for emergency teaching.
