摘  要:目的探讨动态增强磁共振成像(DCE-MRI)的药代动力学信息在乳腺疾病诊断中的临床价值。方法选取34例经DCE-MRI检查证实的BI-RADS 3~6级的女性患者。采用改进的双室Tofts模型和Brix模型获得相应的药代动力学参数。采用Mann-Whitney检验分别比较BI-RADS 3级和BI-RADS 4级、BI-RADS 4级和BI-RADS 5级、BI-RADS 5级和BI-RADS 6级的参数。P<0.05差异有统计学意义。结果在BI-RADS 3级和BI-RADS 4级中Brix-Kep(P=0.008),Abrix(P<0.001)差异有统计学意义。改进的双室Tofts模型和Brix模型获得相应的对比剂动力学参数(除Tofts-Ve)在BI-RADS 4级和BI-RADS 5级差异均有统计学意义。Abrix(AUC=0.904)在BI-RADS 3级和BI-RADS 4级之间具有良好的鉴别能力。Tofts-kep(AUC=0.935)、ABrix(AUC=0.987)对BI-RADS 4级和BI-RADS 5级有较好的辨别能力。结论Brix-Kep、Brix-kel、Abrix、Tofts-ktrans、Tofts-kep、Slope在高级别组(BI-RADS 5级、BI-RADS 6级)中的计算结果高于低级别组(BI-RADS 3级、BI-RADS 4级)肿瘤。Abrix在BI-RADS 3级和BI-RADS 4级之间有很好的辨别能力。Tofts-kep,Abrix对BI-RADS 4级和BI-RADS 5级有很好的辨别能力。所有参数在BI-RADS 5级和BI-RADS 6级之间没有显著差异。Objective To investigate the clinical value of pharmacokinetic information of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in the diagnosis of breast diseases.Methods In this study,34 cases 30~70 years old,mean(47.5±12.3)years old with BI-RADS confirmed by DCE-MRI were retrospectively collected.The corresponding pharmacokinetic parameters were obtained by improved two compartment tofts model and brix model.The parameters of BI-RADS 3 and BI-RADS 4,BI-RADS 4 and BI-RADS 5,BI-RADS 5 and BIRADS 6 were compared by Mann Whitney test.P value less than 0.05 indicated statistical significance.Results There were significant differences in brix kep(P=0.008)and abrix(P<0.001)in BI-RADS 3 and BI-RADS 4.The contrast agent kinetic parameters(except tofts,VE)obtained by the improved two compartment tofts model and brix model were statistically significant in BI-RADS 4 and BI-RADS 5.Abrix(AUC=0.904)had good discrimination ability between BI-RADS 3 and BI-RADS 4.Tofts kep(AUC=0.935)and abrix(AUC=0.987)had good discrimination ability for BI-RADS 4 and BI-RADS 5.Conclusion The results of Brix kep,Brix Kel,abrix,tofts Ktrans,tofts kep and slope in high-grade(BI-RADS 5,BI-RADS 6)tumors are higher than those in low-grade(BI-RADS 3,BI-RADS 4)tumors.Abrix has good discrimination between BI-RADS 3 and BI-RADS 4.Tofts kep and abrix have good discrimination ability to BI-RADS 4 and BI-RADS 5.There is no significant difference in all parameters between BI-RADS 5 and BI-RADS 6.
