摘  要:目的通过检测肺腺癌组织中SPG20甲基化和EGFR水平,探讨二者在肺腺癌发生过程中的相关性。方法选取70例肺腺癌癌组织(实验组)和正常肺组织(对照组),通过焦磷酸测序法检测SPG20基因甲基化水平,采用免疫组化(SP)和Western blot法检测SPG20和EGFR蛋白表达水平;统计分析SPG20甲基化水平与EGFR相关性及其二者与临床病例特征(性别、年龄、TNM分期、肿瘤分化程度、吸烟与否、淋巴结转移与否)的相关性。结果SPG20在癌组织甲基化阳性率为68.57%,高于癌旁正常肺组织(32.86%);免疫组化结果显示EGFR在癌组织中阳性表达为72.86%,显著高于癌旁正常组织的表达(18.57%);肺腺癌组织SPG20阳性率为31.43%,低于癌旁正常肺组织(77.14%),差异有统计学意义(P<0.05);Western blot法实验显示EGFR在癌组织中表达为76.38%±2.67%,高于癌旁组织23.14%±1.86%;SPG20在癌组织中表达为18.42%±1.34%,高于癌旁组织65.75%±2.47%,差异均有统计学意义(P<0.05);肺腺癌组织中SPG20甲基化和EGFR具有相关性(相关系数r=1.00,P<0.05);SPG20甲基化和EGFR与肿瘤TNM分期、组织分化和淋巴结转移密切相关(P<0.05)。结论肺腺癌中SPG20甲基化与EGFR具有明显的相关,二者可能共同促进了肺腺癌的发生和发展。Objective To investigate the correlation between SPG20 methylation and EGFR in the pulmonary adenocarcinoma development process by detecting the SPG20 methylation and EGFR levels in lung adenocarcinoma tissue.Methods Seventy cases of lung adenocarcinoma were selected as the study subjects,including the cancer tissues(experimental group)and normal tissues(control group).The methylation level of SPG20 was detected by pyrophosphate sequencing,and the protein expression level of EGFR and SPG20 was detected by Immunohistochemical(SP)and Western blot.Statistical analysis of correlation between SPG20 methylation level and EGFR protein expression level and the characteristics of clinical cases(Gender,age,TNM staging,degree of tumor differentiation,smoking,lymph node metastasis or not)was performed.Results The positive rate of SPG20 methylation in cancer tissues was 68.57%,which was higher than that in adjacent normal lung tissues(32.86%).Immunohistochemical results showed that EGFR was 72.86%positive in cancer tissues.It was significantly higher than that of normal tissues(18.57%).SPG20 positive rate of lung adenocarcinoma was 31.43%,which was lower than paracancerous normal lung tissue(77.14%),and the differences were statistical significant(P<0.05).Western-Blot results showed that EGFR expressed in cancerous tissues 76.38%±2.67%,higher than paracancerous tissue 23.14%±1.86%;SPG20 expressed in cancerous tissues as 18.42%±1.34%,higher than paracancerous tissue 65.75%±2.47%.All had statistical difference(P<0.05).SPG20 methylation and EGFR in lung adenocarcinoma were correlated(correlation coefficient r=1.00,P<0.05).SPG20 methylation and EGFR were closely associated with tumor TNM staging,tissue differentiation,and lymph node metastasis(P<0.05).Conclusion SPG20 methylation may be associated with the cancer promotion of EGFR,which may promote the occurrence and development of lung adenocarcinoma.
