摘  要:目的:探讨大蒜素对大鼠深Ⅱ度烧伤创面微循环的改善作用。方法:将150只Wistar大鼠随机分为假烧伤组、模型组、大蒜素(低、中、高剂量)组(5 mg·kg^(-1)、10 mg·kg^(-1)、20 mg·kg^(-1)),每组30只。采用加压0.03 MPa、高温106℃持续5 s的方法制备大鼠深Ⅱ度烧伤模型;模型制备成功后,各组腹腔注射给予相应药物,假烧伤组、模型组给予同体积生理盐水,疗程14 d。各组大鼠烧伤创面行肉眼观察及HE染色病理学检查,计算创面愈合率;检测创面组织微循环血流灌注值(microcirculation perfusion valued, MPD)和含水量;测定血小板聚集率,ELISA法检测血清可溶性血管细胞黏附分子1(soluble vascular cell adhesion molecule-1,sVCAM-1)、血管紧张素-Ⅱ(angiotensin-Ⅱ,Ang-Ⅱ)含量;硝酸还原酶法测定血清一氧化氮(nitric oxide, NO)含量;Western blot法检测创伤面内皮素1(endothelin-1,ET-1)、一氧化氮合成酶(nitric oxide synthase, NOS)蛋白表达。结果:与模型组比较,大蒜素各剂量组大鼠创面缩小、创面愈合状况及病理学改变呈不同程度改善;大蒜素中、高剂量组创面愈合率升高,MPD升高而含水量降低,血小板聚集率降低,血清sVCAM-1、Ang-Ⅱ、NO含量降低,创面组织ET-1蛋白表达量降低、NOS蛋白表达量升高,差异均具有统计学意义(P<0.05;P<0.01),且大蒜素高剂量组效果最优。结论:大蒜素能够通过改善局部微循环而促进大鼠深Ⅱ度烧伤创面愈合,其机制可能与降低血小板聚集、抑制内皮细胞黏附、促进血管扩张有关。Objective: To explore the effect of Allicin on alleviating the microcirculation of deep Ⅱ degree burn wounds in rats.Methods: 150 Wistar rats were randomly divided into the pseudo-burn group, model group, and Allicin low-,medium-and high-dose group(5,10,and 20 mg·kg^(-1)),n=30.The rat model of deep Ⅱ degree burn was prepared by pressurization of 0.03 MPa and high temperature of 106 ℃ for 5 s.After the model was successfully prepared, each group was given corresponding drugs by intraperitoneal injection, and the sham burn group and model group were given the same volume of normal saline for 14 days.The burn wounds of rats in each group were observed by visual and HE staining pathological examination, the burn wound healing rate was calculated.The microcirculation blood flow perfusion valued(MPD) and water content in wound tissue was detected.The platelet aggregation rate was measured.And the content of soluble vascular cell adhesion molecule-1(sVCAM-1),angiotensin-Ⅱ(Ang-Ⅱ) in serum was detected by ELISA.The content of nitric oxide(NO) was detected by nitrate reduction.The expression of endothelin-1(ET-1) and nitric oxide synthase(NOS) was determined by the Western blot method.Results: Compared with the model group, the wound reduction, wound healing, and pathological changes of rats in each dose of allicin were improved to varying degrees.In the medium and high-dose allicin groups, the wound healing rate increased, MPD increased but water content decreased.Platelet aggregation rate decreased, and sVCAM-1,Ang-Ⅱ content decreased.NO content increased, wound tissue ET-1 protein expression level decreased, NOS expression increased, and the difference was statistically significant(P<0.05;P<0.01).And the effect of the allicin high-dose group was the best.Conclusion: Allicin can promote the healing of deep second-degree burn wounds in rats by improving local microcirculation.The mechanism may be related to reducing platelet aggregation, inhibiting endothelial cell adhesion, and promoting vasodilation.
