摘 要:目的分析新型冠状病毒肺炎疫情(新冠疫情)期间公立医院医生职业倦怠和工作满意度影响情况,并提出相应的防范措施。方法2021年7月20日至8月10日,以南通市7家市属三甲公立医院医生作为调查对象,设计结构化调查问卷,进行调查与访谈,利用调查数据对其职业倦怠和工作满意度进行定性与定量描述统计和多因素回归分析。结果共回收有效调查问卷381份,241名(63.25%)医生存在职业倦怠,女性医生职业倦怠发生率较男性医生更高(P<0.05);长时间工作、缺少必要的专项激励及较低的薪酬等均是引起医生职业倦怠的因素。职业倦怠、高负荷工作强度、较低的薪酬降低了医生工作满意度。疫情长期化造成医生工作强度及心理负担加重、医患关系矛盾频发是新冠疫情期间引起医生职业倦怠和工作满意度下降的重要因子。结论新冠疫情的爆发给公立医院医生造成了生理和心理负面影响,公立医院及主管部门应高度重视新冠疫情期间医护人员身心健康变化,通过改善其工作环境与条件、加强健康疏导、完善职业激励等措施,维护和提升医生工作积极性和满意度。Objective To evaluate the impact of the COVID-19 pandemic on job burnout and job satisfaction of doctors in public hospitals,so as to put forward corresponding preventive measures.Methods From July 20 to August 10,2021,doctors in 7 municipal grade-A tertiary public hospitals in Nantong city were investigated.A structured questionnaire was designed,and interviews were conducted.Qualitative and quantitative descriptive statistical analysis and multivariate regression analysis were conducted to analyze the job burnout and job satisfaction of doctors during the COVID-19 pandemic.Results A total of 381 valid questionnaires were collected.The survey results showed that during the COVID-19pandemic,241 doctors(63.25%)had job burnout,and the incidence of job burnout in female doctors was higher than that in male doctors(P<0.05).Long working hours,lack of necessary special incentives and low salary were the factors leading to job burnout in doctors.Job burnout,high workload and low salary reduced job satisfaction in doctors.The increased work intensity and psychological burden of doctors and frequent conflicts between doctors and patients caused by the prolonged epidemic were important factors leading to job burnout and decreased job satisfaction in doctors during the COVID-19 pandemic.Conclusion The COVID-19 pandemic has had a negative physical and psychological impact on doctors in public hospitals.Public hospitals and administrative departments should attach great importance to the changes in physical and mental health of medical staff during the COVID-19 pandemic,and maintain and enhance their enthusiasm and job satisfaction by improving their working environment and conditions,strengthening health counseling and improving occupational incentives.
