珍枣胶囊干预老年阴虚火旺型失眠患者中医证素的演变规律及其与神经递质关联性的研究

摘　　要：目的:观察珍枣胶囊治疗老年阴虚火旺型失眠患者证素演变规律和作用机制。方法:选取120例符合标准的患者,随机分为治疗A组和治疗B组,每组60例。治疗A组患者给予珍枣胶囊治疗,治疗B组选择给予百乐眠胶囊治疗。治疗4周后,观测治疗前后血浆5-羟色胺(5-HT)和多巴胺(DA)水平的变化及其与病位证素的相关性,并对治疗前后患者的临床疗效、证素演变规律以及中医证候疗效进行评定。结果:两组临床疗效相当,但中医证候疗效方面,与治疗B组比较,治疗A组改善更明显,P<0.05。两组患者治疗前后相比,5-HT水平皆提高且DA水平皆降低,治疗A组调节幅度优于治疗B组,均P<0.05。治疗A组药物对心证素、肝证素的作用明显优于治疗B组药物,P<0.05;两组药物对肾证素的干预作用无显著差异性,P>0.05。结论:珍枣胶囊能改善老年阴虚火旺型失眠患者的中医证候,其对病位证素疗效显著,具有良好的临床效果。Objective:To observe the change of syndrome factors and mechanism of Zhen Zao Capsule in the treatment of senile patients with yin-deficiency and fire-excess insomnia.Methods:120 cases were randomly divided into treatment group A and treatment group B,60 cases in each group.The treatment group A patients were given Zhen Zao Capsule half an hour before sleep,and the treatment group B patients were treated with Bailemian Capsule at the same time.After the treatment of 4 weeks,the relevance between insomnia's factor in TCM and the relevant neurotransmitters such as 5-hydroxytryptamine(5-HT)and dopamine(DA)was explored,as well as the clinical effects and the law of evolution about syndrome factor of two groups were evaluated.Results:The clinical effects of both groups were equal.However,in terms of improving the TCM syndromes and effects,the treatment group A was superior to the treatment group B(P<0.05).Before and after treatment,5-HT increased while DA decreased.Amplitude of accommodation in the treatment group A was significant better than that of the treatment group B(P<0.05),especially in the symptom factor of liver and heart.Both groups had no significant difference on the effect of the symptom factor of kidney(P>0.05).Conclusion:With significant clinical effects,Zhen Zao Capsule can improve insomnia in senile patients;it has a remarkable curative effect on syndrome factor.It has achieved a good clinical effect.
