组织结构和细胞形态变异对诊断硬化性肺细胞瘤的影响

摘　　要：目的探讨硬化性肺细胞瘤组织结构和细胞形态变异的特征。方法收集281例硬化性肺细胞瘤患者的临床资料,采用HE、免疫组化SP法染色,根据免疫组化结果对存在变异的组织结构和细胞形态予以确认。结果281例硬化性肺细胞瘤中有2例在同一肺叶内出现多发病灶,4例伴局部淋巴结侵袭或转移,1例合并典型类癌。同时出现4种基本结构的病例仅占17.08%,以单一结构为主的病例占30.25%(实性为主28例、乳头状为主37例、血管瘤样为主20例),大多数的硬化性肺细胞瘤表现为2~3种混合型组织结构,占52.67%(148/281)。约1/3病例出现组织结构和细胞形态变异:多核瘤巨细胞形成、模拟乳头或微乳头结构、硬化性乳头、间质细胞的梭形变和透明变、间质的黏液变以及与其它恶性肿瘤共存等。结论硬化性肺细胞瘤的组织结构复杂,且其组织结构和细胞形态常出现变异。Purpose To explore the variations of tissue structure and cell morphology of sclerosing pneumocytoma.Methods The clinical data of 281 cases of sclerosing pneumocytoma were collected.The tissue structure and cell morphology variations were confirmed by immunohistochemistry.Results Among 281 cases,2 cases had multiple lesions in the same lung lobe,4 cases had local lymph node invasion or metastasis,and 1 case had typical carcinoid.Only 17.08%of the cases had four basic structures concurrently.The cases with single structure accounted for 30.25%(28 cases were solid,37 cases were papillary,20 cases were hemangioma-like).Most sclerosing pneumocytoma showed mixed tissue structures of 2-3 structures,accounting for 52.67%(148/281).About 1/3 of the cases showed variations in tissue structure and cell morphology,such as multinucleated giant cell formation,mimiking papillary or micropapillary structure,sclerosing papilla,spindle and clear change of stromal cells,interstitial mucoid degeneration and coexistence with other malignant tumors.Conclusion The tissue structure of sclerosing pneumocytoma is complex,and there are often variations in tissue structure and cell morphology.
