婴儿痉挛患儿促肾上腺皮质激素治疗前后免疫水平的变化

摘　　要：目的:比较婴儿痉挛患儿激素治疗前后T细胞亚群、免疫球蛋白及细胞因子的水平,探讨免疫学在婴儿痉挛发病机制及治疗中的重要作用。方法:分别采集健康婴儿及婴儿痉挛患儿促肾上腺皮质激素治疗前后的空腹静脉血,测定CD3+、CD4+、CD8+、细胞间黏附分子-1(ICAM-1)、IgA、IgG、IgM、白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α的水平,并进行比较分析。结果:与健康婴儿相比,病例组患儿治疗前CD3+、CD4+、IgA水平显著降低,CD8+、ICAM-1、IgG、IL-6、TNF-α水平显著增高(P<0.05);治疗3个月后,CD4+、IgA水平显著增高,CD8+、ICAM-1及IgG水平显著降低(P<0.05);与治疗前比较,病例组患儿治疗3个月后IgA水平显著增高,CD8+、ICAM-1及IgG水平显著降低(P<0.05)。结论:婴儿痉挛患儿存在免疫功能紊乱,激素治疗后血清学免疫指标发生变化,进而从发病机制及治疗角度支持免疫因素在婴儿痉挛中的重要性。Objective:To compare the levels of T cell subsets,immunoglobulins and cytokines before and after adrenocorticotropic hormone therapy in children with infantile spasm,so as to explore the important role of immunology in the pathogenesis and treatment of infantile spasm.Methods:Fasting venous blood before and after adrenocorticotropic hormone therapy was collected from healthy infants and children with infantile spasm,and the contents of CD3+,CD4+,CD8+,intercellular adhesion molecule-1(ICAM-1),IgA,IgG,IgM,interleukin(IL)-6 and tumor necrosis factor(TNF)-αwere measured and compared.Results:Compared with healthy infants,the levels of CD3+,CD4+and IgA decreased significantly,while the levels of CD8+,ICAM-1,IgG,IL-6 and TNF-αincreased significantly in the case group before treatment(P<0.05).After treatment of 3 months,the levels of CD4+and IgA increased significantly,while the levels of CD8+,ICAM-1 and IgG decreased significantly(P<0.05).Compared with before treatment,the levels of IgA in the case group increased significantly,and the levels of CD8+,ICAM-1,IgG decreased significantly after treatment of 3 months(P<0.05).Conclusion:Children with infantile spasms have immune dysfunction,and the serological immune indicators have changed after hormone therapy,which further supports the importance of immune factors in infantile spasm from the perspective of pathogenesis and treatment.
