摘 要:目的:研究青少年学生总胆红素(TBil)升高情况并分析其危险因素,为探查青少年黄疸原因提供思路。方法:通过对北京市某高校2019年319名新生的健康体检资料相关数据进行分析,计算TBil升高率,并用Logistic回归分析TBil升高的相关因素。结果:青少年学生TBil升高率为12.54%(40/319),TBil升高率男性明显大于女性(19.08%vs 6.59%,P<0.05)。单变量Logistic回归结果示男性、低体重是TBil升高的危险因素(P<0.05)。年龄、学制、丙氨酸氨基转移酶(ALT)不是TBil升高的危险因素。多自变量Logistic回归结果显示男性、低体重均是TBil升高的危险因素,无混杂因素,在其他因素不变的情况下,青少年男学生胆红素升高发生率是女学生的3.33倍,低体重者胆红素升高发生率是非低体重者的2.33倍。结论:青少年学生TBil升高,可能与肝损伤不相关,而男性、低体重是TBil升高的危险因素。Objective:To study the elevation of total bilirubin(TBil) in adolescent students and analyze its risk factors,so as to provide clinicians with ideas for the diagnosis and treatment of adolescent jaundice.Methods:Through the analysis of the health examination data of 319 freshmen in a university in Beijing in 2019,the TBil increase rate was calculated,and the chi-square test,univariate and multivariate Logistic regression were used to analyze the related factors of TBil increase.Results:The increase rate of TBil among adolescent students was 12.54%(40/319),and the chi-square test showed that the increase rate of TBil in men(19.08%) was significantly higher than that in women(19.08% vs 6.59%,P<0.05).Univariate logistic regression results showed that male sex was a risk factor for elevated TBil,and low body mass index(BMI) was a risk factor for elevated TBil(P<0.05).Age,program duration,and Alanine aminotransferase(ALT) were not risk factors for elevated TBil.The multivariate Logistic regression showed that the male and underweight were both risk factors for elevated TBil.There was no confounding factor.With other factors unchanged,male adolescent students had 3.33 times the increase in bilirubin compared with females.The increase in bilirubin in the underweight was 2.33 times that of the non-underweight.Conclusion:The high rate of elevated TBil among adolescent students may not be related to liver injury,while the male and the underweight were risk factors for elevated TBil.
