摘　　要：本文利用Meta分析探讨GPC-3、AFP单项检测及GPC-3+AFP联合检测对原发性肝细胞肝癌的诊断价值。在PubMed、Webof Science和Elsevier等三大数据库中按照纳入排除标准筛选文献,用QUADAS-2评价纳入的文献,并运用Meta-Disc 1.4、Stata 12.0和Review Manager 5.3等软件对纳入文献进行分析,最终获得14篇文献。亚组分析表明发表时间、样本量、研究地址均不是异质性来源;通过发表偏倚检测和敏感性分析表明所得结果是稳定可靠的;通过GPC-3+AFP联合检测与其各单项检测效能进行比较分析可知,联合检测的敏感度、特异度、阳性似然比、阴性似然比、诊断比值比、AUC均优于单项检测;联合检测提高了原发性肝癌的检出能力,可为后续肝癌的临床诊断提供借鉴。The diagnostic value of GPC-3,AFP single and GPC-3+AFP combined detection for primary hepatocellular carcinoma was investigated by Meta-analysis.Firstly,the literature of PubMed,Web of Science and Elsevier was screened according to the inclusion and exclusion criteria.Secondly,the included literature was assessed by QUADAS-2 and analyzed by Meta-Disc 1.4,Stata 12.0 and Review Manager 5.3.Finally,14 literature was included.Subgroup analysis showed that publication time,sample size and research address were not the source of heterogeneity.Publication bias detection and sensitivity analysis showed that the results were stable and reliable.The detection efficiency comparison showed that the sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,diagnostic odds ratio and AUC of GPC-3+AFP combined detection were better than those of single detection.Combined detection can improve the detectability of primary hepatocellular carcinoma and provide reference for clinical diagnosis of liver cancer.
