摘  要:目的 探讨肺癌组织中p53基因点突变情况与转移和预后的相关性。方法 选取2016年8月至2020年9月间河南大学附属南阳南石医院收治的90例非小细胞肺癌患者的病理组织,根据p53基因点是否突变分为点突变组18例和非点突变组72例。采用聚合酶链式反应(PCR)方法检测p53基因点突变情况,随访患者的转移和预后情况并进行相关性分析。结果 点突变组患者转移率为44.4%,近期总有效率为55.6%;非点突变组患者转移率为15.3%,近期总有效率为84.7%,两组比较,差异均有统计学意义(均P<0.05)。90例患者中,Spearsman分析显示,p53基因点突变与预后转移和总有效率均存在相关性,差异有统计学意义(均P<0.05)。多因素Cox比例风险模型筛选显示,p53基因点突变为影响患者预后转移和总有效率的重要因素,差异有统计学意义(均P<0.05)。受试者工作特征(ROC)曲线显示,p53基因点突变预测预后转移和总有效率的曲线下面积分别为0.789和0.797。结论 肺癌患者多伴有p53基因点突变,与患者的转移和预后存在相关性,是预测转移和预后的重要指标。Objective To explore the association between point mutation in p53 gene and metastases and prognosis in lung cancer tissues. Methods From August 2016 to September 2020,pathological tissues were collected from 90 patients with non-small cell lung cancer at Nanyang Nanshi Hospital Affiliated to Henan University. Based on whether point mutation occurred in p53 gene detected by polymerase chain reaction(PCR), they were categorized into a point mutation group(18 patients) and a non-point mutation group(72 patients). Patients were followed up for metastasis and prognosis, and the association was analyzed. Results The incidence of metastases was 38.9% and the short-term efficacy was 55.6% in the point mutation group and 15.3% and 84.7%, respectively in the non-point mutation group(all P<0.05). Spearman analysis showed point mutation in p53 gene were associated with prognosis, metastasis and overall efficacy rate(all P<0.05). Multivariate Cox proportional hazards model screening showed that point mutation in p53 gene were important factor affected the prognosis, metastasis and overall response rate(all P<0.05). The receiver operating characteristic(ROC) curve showed that the area under the curve of point mutation in p53 for the prediction of prognosis, metastasis and overall efficacy rate was 0.789 and 0.797, respectively. Conclusion Lung cancer patients are mostly accompanied by point mutation in p53 gene, which is associated with the metastasis and prognosis, and it is also an important indicator for predicting the metastasis and prognosis in lung cancer.
