摘 要:目的 探讨胃癌组织中程序性死亡配体-1(PD-L1)及间充质-上皮转化因子(c-Met)临床病理特征、预后意义及二者相关性。方法 收集我院2014年临床资料完整的病例共208例。应用免疫组织化学的方法检测胃癌组织中PD-L1及c-Met的表达,分析其与临床病理特征之间的关系以及二者之间的相关性。结果 胃癌组织中PD-L1-TPS阳性率为14.9%,PD-L1-CPS阳性率为37.0%,女性中PD-L1-TPS阳性率(25.5%)高于男性(11.1%),差异具有统计学意义(P<0.05)。c-Met高表达率为37.5%,脉管瘤栓阳性组(47.1%)高表达率高于脉管瘤栓阴性组(34.7%),c-Met高表达患者的5年总生存率低于c-Met低表达者,分别为42.5%及64.6%,差异有统计学意义(P<0.05)。胃癌组织中PD-L1及c-Met的表达存在正相关(P<0.05)。结论 c-Met与PD-L1在胃癌中表达,其中c-Met与脉管瘤栓及预后差相关。胃癌组织中PD-L1及c-Met的表达存在相关性,这为未来胃癌免疫治疗联合靶向治疗提供依据。Objective To investigate the clinicopathological features, prognostic significance and correlation of programmed cell death ligand 1(PD-L1) and mesenchymal epithelial transition factor(c-Met) in gastric cancer. Methods A total of 208 cases with complete clinical data in our hospital in 2014 were collected. The expression of PD-L1 and c-Met in gastric cancer was detected by immunohistochemistry, and the relationship between PD-L1 and c-Met and clinicopathological features and the correlation between them were analyzed. Results The positive rate of PD-L1-TPS and PD-L1-CPS in gastric cancer was 14.9% and 37.0%, respectively. The positive rate of PD-L1-TPS in women(25.5%) was higher than that in men(11.1%). The difference was statistically significant(P<0.05). The high expression rate of c-Met was 37.5%. The high expression rate of vascular tumor thrombus positive group(47.1%) was higher than that of negative group(34.7%). The 5-year overall survival rate of patients with high expression of c-Met was lower than that of patients with low expression, 42.5% and 64.6%, respectively. The difference was statistically significant(P<0.05). There was a positive correlation between the expression of PD-L1 and c-Met(P<0.05). Conclusion c-Met and PD-L1 are expressed in gastric cancer, and c-Met is associated with vascular tumor thrombus and poor prognosis. The expression of PD-L1 and c-Met in gastric is correlated, which provides a basis for immunotherapy combined with targeted therapy for gastric cancer in the future.
