摘 要:目的探索基于模拟体验系统的VAD教学法在关节镜手术模拟教学中的应用效果。方法选取2021年4—5月在陕西省人民医院参加住院医师规范化培训的120名学员,按照随机对照原则分为试验组(n=60)与对照组(n=60)。试验组接受基于模拟体验系统的VAD教学法,对照组接受单纯VAD教学法;培训周期均为4周。培训结束后对所有学员完成关节镜下各项操作的效率进行评估。结果两组学员在年龄、性别、优势手、专业及既往有无关节镜操作史方面的差异无统计学意义(P>0.05);试验组完成膝关节镜下标准化诊断操作的评分为(63.0±4.4),显著高于对照组的(38.0±5.2)分,差异具有统计学意义(P<0.05);试验组完成膝关节镜下游离体取出和套环操作的评分为(23.0±3.8),也显著高于对照组的(14.0±4.5),差异具有统计学意义(P<0.05);在最终的DASS评分方面,试验组的得分为(86.0±5.2),高于对照组的(48.0±5.8)分,差异具有统计学意义(P<0.05)。结论模拟体验系统与VAD相结合的教学方法可有效提高住培学员的关节镜操作技能,提升住培学员对关节镜下实践操作的学习效率。Objective To evaluate the application of video-assisted debriefing(VAD)teaching method based on simulation experience system in simulation teaching of arthroscopic surgery.Methods Between April 2021 to May 2021,120 students who were enrolled in the standardized residents training program in Shaanxi Provincial People's Hospital were chosen to investigate the training experience.All students were randomly divided into the experimental group(n=60)and the control group(n=60).The experimental group was taught by VAD teaching method based on simulation experience system,while the control group was taught by VAD teaching method only.After four weeks of training,students'efficiencies in arthroscopic skills were compared.Results There was no significant statistical difference between the age,sex,dominant hand,profession and the history of operating arthroscopic surgery between the two groups(P>0.05).In the stand-ardized diagnostic operation of knee arthroscopy,the experimental group obtained significantly higher score(63.0±4.4)than the control group(38.0±5.2),and the difference was statistically significant(P<0.05).In addition,students in the experimental groups acquired higher scores(23.0±3.8)than the control group both in catching loose body and hitching rings sections(14.0±4.5),and the difference was statistically significant(P<0.05).In terms of the final DASS evaluation,scores were significantly higher in the experimental group(86.0±5.2)than in the control group(48.0±5.8),and the difference was of statistical significance(P<0.05).Conclusion The teaching method combining simulation experience system and VAD can effectively improve resi-dents'arthroscopic skills and promote their learning efficiency in practical operation with arthroscopy.
