高金合金在口腔修复中的应用效果及对口腔局部健康指数、龈沟液细胞因子的影响研究

摘　　要：目的:探究及观察高金合金在口腔修复中的应用效果及对口腔局部健康指数、龈沟液细胞因子的影响。方法:选取2019年3月-2021年2月临沂市人民医院收治的100例口腔修复患者,根据随机数字表法分将其为对照组和观察组,每组50例。对照组采用镍铬合金进行修复,观察组采用高金合金进行修复。比较两组边缘密合度、颜色、颈缘染色及崩瓷发生情况;比较两组口腔局部健康指数(牙龈指数、龈沟出血指数及菌斑指数)及龈沟液细胞因子[白细胞介素-1β(IL-1β)、白细胞介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)]。结果:治疗后1、6个月,观察组边缘密合度及颜色情况均优于对照组,差异均有统计学意义(P<0.05)。观察组不良情况发生率低于对照组,差异有统计学意义(P<0.05)。治疗前,两组牙龈指数、龈沟出血指数、菌斑指数、IL-1β、IL-8、TNF-α水平比较,差异均无统计学意义(P>0.05);治疗后1、3个月,观察组牙龈指数、龈沟出血指数、菌斑指数、IL-1β、IL-8、TNF-α水平均低于对照组,差异均有统计学意义(P<0.05)。结论:高金合金在口腔修复中的临床效果较好,且对口腔局部健康指数及龈沟液细胞因子表达的不良影响也相对较小,因此在口腔修复患者中的应用价值较高。Objective:To investigate and observe the application effect of high gold alloy in prosthodontics and influence on oral local health index and gingival crevicular fluid cytokines.Method:A total of 100 patients with prosthodontics in Linyi People’s Hospital from March 2019 to February 2021 were selected,and they were divided into control group and observation group according to random number table,with 50 cases in each group.The control group were treated with Ni-Cr alloy,the observation group were treated with high gold alloy.The edge consistency,color,neck staining and the occurrence of porcelain fracture were compared between two groups;oral local health index(gingival index,gingival sulcus bleeding index and plaque index)and gingival crevicular fluid cytokines[interleukin-1β(IL-1β),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)]were compared between two groups.Result:1 month and 6 months after treatment,the edge consistency and color in the observation group were better than those in the control group,the differences were statistically significant(P<0.05).The incidence of adverse events in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in gingival index,gingival sulcus bleeding index,plaque index,IL-1β,IL-8 and TNF-αbetween two groups(P>0.05);1 month and 3 months after treatment,the gingival index,gingival sulcus bleeding index,plaque index,IL-1β,IL-8 and TNF-αlevels in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The clinical effect of high gold alloy in prosthodontics is better,and there is less bad influence for oral local health index and gingival crevicular fluid cytokines expression,so its application value in patients with prosthodontics is higher.
