摘 要:目的了解北京市腹泻病人分离的小肠结肠炎耶尔森菌的病原学特征。方法对腹泻病人粪便样本中分离到的小肠结肠炎耶尔森菌进行血清分型、生物分型、毒力基因检测、核心基因组多位点序列分型(core genome multilocus sequence typing,cgMLST)、耐药分析以及脉冲场凝胶电泳(pulsed-field gel electrophoresis,PFGE)分子分型试验。结果2016-2019年从腹泻病人中共检出8株小肠结肠炎耶尔森菌,其中生物血清型3/O∶3型5株,1A/血清未定型3株;毒力基因检测可分为三种类型,致病株毒力基因分布为ail+、ystA+、ystB-、yadA+、virF+、foxA+、rfbC+;cgMLST显示有4株致病性菌株为cgST2571型,1株致病性菌株为cgST2572型,另3株非致病性菌株分别为三种其他型别。药敏试验结果显示,8株菌对头孢唑林、氨苄西林和磺胺异噁唑耐药率较高,且有7株存在多重耐药现象;经NotⅠ酶切后,8株菌共分为6种PFGE带型。结论北京市腹泻病人分离的小肠结肠炎耶尔森菌主要为生物血清型3/O∶3型致病性菌株,多重耐药现象比较严重,PFGE带型呈多态性分布。Objective To analyze the pathogenic characteristics of Yersinia enterocolitica isolated from diarrhea patients in Beijing.Methods 8 Yersinia enterocolitica strains isolated from diarrhea patients were analyzed by serotyping,bioserotyping,virulence genes detection,core genome multilocus sequence typing(cgMLST),antimicrobial resistance test and pulsed-field gel electrophoresis(PFGE)for molecular typing.Results There were 8 strains of Yersinia enterocolitica isolated from diarrhea patients during 2016-2019,including bioserotype 3/O∶3(5 strains)and biotype 1A with an unknown serotype(3 strains).Three patterns of virulence genes were identified.The virulence gene distribution of pathogenic strains was ail+,ystA+,ystB-,yadA+,virF+,foxA+,rfbC+.The result of cgMLST showed that 4 pathogenic strains were cgST2571,1 pathogenic strain was cgST2572,and the other 3 nonpathogenic strains were three other different patterns.Antimicrobial resistance test suggested that 8 strains were high resistance to cefazolin,ampicillin and sulfisoxazole,and 7 strains showed multi-drug resistance.After NotⅠdigestion,8 strains were divided into 6 band types.Conclusions Yersinia enterocolitica isolated from diarrhea patients in Beijing were mainly pathogenic bioserotype 3/O∶3 strains.These strains showed high multi-drug resistance.And the PFGE band types were polymorphically distributed.
