摘　　要：目的:分析儿童青少年幕上脑实质内囊实性肿瘤行磁共振成像(MRI)的诊断价值。方法:收集开封市中心医院及山东第一医科大学附属省立医院2014年6月至2021年5月期间经手术及病理确诊的46例幕上脑实质内囊实性肿瘤儿童青少年的临床资料,回顾性分析其MRI影像表现特征。结果:所有病灶均为囊实性表现,46例肿瘤中,神经节细胞胶质瘤(GG)13例、毛细胞星形细胞瘤(PA)8例、多形性黄色星形细胞瘤(PXA)9例、胚胎发育不良性神经上皮肿瘤(DNET)4例、室管膜瘤8例、不典型畸胎样/横纹肌样瘤(AT/RT)3例、婴幼儿促纤维增生型肿瘤(DIT)1例。结论:儿童青少年幕上脑实质内囊实性肿瘤发病率较低,临床症状无特异性,MRI检查能较好的显示肿瘤的部位、大小、形态与边界、信号特点、囊壁与实性成分强化特征、瘤周水肿、占位效应、转移情况,对于肿瘤的典型影像学表现有一定的特征性,但是表现不典型时易被误诊,认识并掌握其MRI表现特点,有助于提高诊断准确率。Objective To analyze the diagnostic value of magnetic resonance imaging (MRI) for supratentorial intraparenchymalcystic solid tumor in children and adolescents.Methods The clinical date of 46 supratentorial parenchymal cystic solid tumor of children and adolescents admitted to Kaifeng Central Hospital and Provincial Hospital Affiliated to Shandong First Medical University from June 2014 to May 2021 were collected,and MRI features were analyzed retrospectively.Results All lesions were cystic and solid.Among the 46 tumors,ganglioglioma (GG) 13 cases,pilocytic astrocytoma (PA) 8 cases,pleomorphic xanthoastrocytoma (PXA) 9 cases,dysembryoplastic neuroepithelial tumors (DNET) 4 cases,ependymoma 8 cases,atypical teratoid/rhabdoid tumor (AT/RT) 3 cases,desmoplastic infantile tumor (DIT) 1 case.Conclusion The morbidity of supratentorial brain parenchymal cystic solid tumors of children and adolescents is low and clinical symptoms are not specific.MRI can better display the location,size,morphology and boundary of tumor,signal characteristics,enhancement characteristics of cyst wall and solid components,peritumoral edema,mass effect and metastasis,typical imaging manifestations of tumors have certain characteristics,but the untypical manifestations are easy to be misdiagnosed.Understanding and mastering the MRI features are helpful for improving of diagnostic accuracy.
