摘  要:目的:探讨肺鳞状细胞癌(鳞癌)和腺癌PD-L1蛋白及相关mi RNA表达的差异。方法:2019年5月至2020年11月来我院就诊的非小细胞肺癌初治患者纳入本项研究;按照病理类型,将患者分为腺癌组和鳞癌组;H&E染色检测免疫细胞数量;免疫组化检测PD-L1、ki-67、PD-1、CTLA-4和LAG-3的表达;mi RNA测序筛选鳞癌和腺癌间差异表达的mi RNA。结果:H&E染色结果显示鳞癌组微环境中免疫细胞的数量为86.86±8.96个/高倍视野(HPF),腺癌组的数量为26.29±3.99个/HPF(t=6.173,P<0.001);肺鳞癌组微环境免疫细胞PD-1、CTLA-4和LAG-3阳性表达的比例分别为53.71±6.88%、35.29±3.25%和34.43±3.29%,腺癌组阳性表达的比例分别为22.29±3.80%、13.43±2.32%和24.00±1.98%(t=3.997,P=0.002;t=5.476,P<0.001;t=2.719,P=0.019);肺鳞癌组患者PD-L1蛋白阳性表达的比例为76.67%,腺癌组的比例为36.67%(P=0.001);肺鳞癌PD-L1(mi R-135、mi R-24和mi R-30b等)和PD-1(mi R-802、mi R-155和mi R-3127-5p等)相关mi RNA的表达均显著高于腺癌。结论:肺鳞癌PD-L1蛋白及相关mi RNA的表达、微环境免疫细胞PD-1、CTLA-4和LAG-3阳性比例均显著高于腺癌。Objective:To investigate the differences of PD-L1 protein and related mi RNA expression between lung squamous cell carcinoma and adenocarcinoma.Methods:From May 2019 to November 2020,newly treated patients with non-small cell lung cancer in our hospital were included in this study.According to the pathological types,the patients were divided into adenocarcinoma group and squamous cell carcinoma group.H&E staining was used to detect the number of immune cells.PD-L1,Ki-67,PD-1,CTLA-4 and LAG-3expression was detected by immunohistochemistry.The differentially expressed mi RNAs between squamous cell carcinoma and adenocarcinoma were screened by mi RNA sequencing.Results:H&E staining results showed that the number of microenvironment immune cells in squamous cell carcinoma group was 86.86±8.96/HPF,and that in adenocarcinoma group was 26.29±3.99/HPF(t=6.173,P<0.001).The positive expression rates of PD-1,CTLA-4 and LAG-3 in microenvironment immune cells of lung squamous cell carcinoma group were 53.71±6.88%,35.29±3.25% and 34.43±3.29%,respectively,and those of adenocarcinoma group were 22.29±3.80%,13.43±2.32% and 24.00 ±1.98%(t=3.997,P=0.002;t=5.476,P<0.001;t=2.719,P=0.019).The positive expression of PD-L1 protein was76.67% in lung squamous cell carcinoma group and 36.67% in adenocarcinoma group(P=0.001).The expression of mi RNAs associated with PD-L1(mi R-135,mi R-24 and mi R-30b,etc.) and PD-1(mi R-802,mi R-155 and mi R-3127-5p,etc.) in lung squamous cell carcinoma was significantly higher than that in adenocarcinoma.Conclusion:The expression of PD-L1 protein and related mi RNA in lung squamous cell carcinoma,the positive proportion of PD-1,CTLA-4 and LAG-3 in microenvironment immune cells were significantly higher than those in adenocarcinoma.
