摘 要:目的探讨基于翻转课堂的模拟训练和临床实践教学在骨科住培急诊手术能力培训中的意义和影响。方法研究采用前瞻性研究方法,选取我院2018年9月—2020年8月期30例骨科住院医师,随机分为两组,观察组和对照组,两组均为15例,培训周期均为1年。观察组是对本组住院医师使用翻转课堂形式进行模拟培训和临床实践教学,对照组为住院医师接受传统教学方法。在培训前、培训后6月及12月时,本研究通过操作技能直接观察评估(directly observed procedural skills,DOPS)对观察组和对照组的住院医师进行考核。结果培训前,观察组中理论成绩和模拟训练DOPS结果高于对照组,但两组之间无统计学意义(均P>0.05)。与对照组相比,在6月和12月时,观察组学员成绩提高更明显,两组之间均有统计学差异(P<0.05)。结论基于翻转课堂的模拟训练和临床实践教学提高了骨科住院医师急诊手术实践能力,可以缩短骨科住院医师急诊手术实践能力的学习周期。Objective To explore the significance and impact of simulation training and clinical practice by flipped classroom teaching on the emergency surgery competence of orthopedic residents.Methods Prospective research methods were used in this study,selecting 30 orthopedic residents in our hospital from September 2018to August 2020,who were randomly divided into two groups,the experimental group and the control group with 15residents in each group.The training period was 1 year.In the experimental group,the faculty members used the flipped classroom format for simulated training and clinical practice teaching,The control group was residents receiving traditional teaching methods.Before training,6 months after training,and 12 months after training,the residents in the study group and the control group were assessed by Directly Observed Procedural Skills(DOPS)in both simulation training and clinical emergency surgical practice skills.Results Before training,theoretical scores and simulation training DOPS results were higher in the study group than in the control group,but there was no statistical significance between the two groups(P>0.05).Compared with the pre-training period,as the training time gradually increased,the DOPS results of simulation training and clinical practice in both groups improved to varying degrees,with significantly differences(P<0.05).Compared with the control group,the participants’performance improved more obviously in the experimental group after 6 and 12 months,with statistically significant differences between two groups(P<0.05).Conclusion Flipped classroom-based simulation training and clinical practice teaching improved the practical ability of emergency surgery for orthopedic residents.This teaching method can shorten the learning period of emergency surgical practice ability of orthopaedic residents.
