摘 要:目的探究基于以资源为基础的相对价值比率(resourcebased relative value scale,RBRVS)的绩效管理方案一,RBRVS和疾病诊断相关分组(diagnosis related group,DRG)相结合的绩效管理方案二的优缺点,以及哪种方案更符合医院的发展趋势。方法运用描述统计和多元线性回归分析法,选取医院外1科、外17科、内15科三个代表科室,分析其2020年9—11月临床医师两种方案的绩效数据。结果两种方案的直接绩效、间接绩效、成本均有不同程度的变化,在直接工作绩效中,两种方案都能体现多劳多得,但方案二能体现更“优”的多劳多得;在间接工作绩效中方案二更能体现“优劳优酬”,成本上方案二人力成本的增加也促使科室建立更“优”的人事管理。结论RBRVS与DRG相结合的绩效管理方案二更能体现“多劳多得、优绩优酬”,更符合医院绩效管理趋势。Objective To explore the advantages and disadvantages of performance management scheme 1 based on resource-based relative value scale(RBRVS),and performance management scheme 2 combining RBRVS and disease diagnosis related group(DRG),and which scheme is more in line with the development trend of the hospital.Methods Descriptive statistics and multiple linear regression analysis were used to analyze the performance data of two programs for clinicians from September to November 2020in three representative departments:1st department of external medicine,17th department of external medicine and 15th department of internal medicine.Results The direct performance,indirect performance and cost of the two schemes all changed to different degrees.In the direct performance,the two schemes could reflect more work and more get,but the second scheme could reflect more"excellent"more work and more get.In indirect work performance,scheme 2 could better reflect"excellent labor and excellent reward".In terms of cost,the increase of manpower cost of scheme 2 also promoted the department to establish more"excellent"personnel management.Conclusion RBRVS and DRG combined performance management scheme 2 can better reflect"more work,more reward,excellent performance",more in line with the trend of hospital performance management.
