我国医养结合服务模式实践逻辑与协同路径分析:基于“全国医养结合典型案例”的扎根理论研究

摘　　要：文章在对我国医养结合的政策背景进行回顾的基础上,对传统的嵌入理论框架进行适应性调整后构建了一个简单的“需求—资源—供给”嵌入框架,运用扎根理论对《全国医养结合典型经验案例》中的实践经验,从“养中有医”“医中有养”和“医养共体”三个角度深入剖析了我国医养结合服务的实践逻辑及其传导机制。研究发现,三个维度从结构嵌入和功能嵌入两个层面形成了多个逻辑链条,并在优化资源配置的视角下呈现了以“养中有医”为主体,“三养共享”“以医促养”的两种传导机制。研究认为,我国未来在医养结合服务模式的优化和调整上,应当把握不同模式的特点,积极探寻其主体行为逻辑和实践行为逻辑,在优化现有模式的基础上,拓宽关联资源的协同路径,实现医养结合服务在模式和实践路径上的创新。Based on the review of the policy background of the combination of medical care and maintenance in China,this paper constructs a simple“demand-resource-supply”embedded framework after adaptively adjusting the traditional embedded theoretical framework,and uses the grounded theory to analyze the practical experience in the national typical experience case of the combination of medical care and maintenance,from“there is a doctor in the combination of medical care”This paper deeply analyzes the practical logic and transmission mechanism of medical support combined service in China from the perspectives of“medical support”and“medical support community”.It is found that the three dimensions form multiple logical chains from the two levels of structural Embeddedness and functional embeddedness,and present two transmission mechanisms of“supporting Chinese medicine”as the main body,“sharing three supports”and“promoting maintenance with medicine”from the perspective of optimizing resource allocation.The research shows that China should grasp the characteristics of different modes,actively explore their main behavior logic and practical behavior logic,broaden the collaborative path of related resources on the basis of optimizing the existing mode,and realize the innovation of integrated medical care service in the mode and practice path.
